- Please complete the attached bid schedule with pricing and return it with you quote. This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This procurement is being issued as full and open competition. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-78 effective 26 Dec 14 and DFARS Change Notice 20141216. The North American Industry Classification System (NAICS) code is 334510 and the size standard is 500 employees. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response. Award will be made in accordance with FAR Part 12, Acquisition of Commercial Items using Simplified Acquisition Procedures in FAR 13.106. REQUIREMENT/SALIENT CHARACTERISTICS: This is a brand name or equal requirement for the Acoustics System RS-252 Single Wall Control / Double Wall Exam Suite with installation at Joint Base San Antonio, Lackland to perform basic trainee hearing exams and for the Otometrics 10-man double wall hearing booth (or equal) with installation at Maxwell Air Force Base, AL. The RS-252 requirement consists of multi-station double wall booths that provide additional acoustic control to isolate patients from waiting room noise. Must have an isolated acoustic floor, acoustic ventilation system, jack panels at each test position for left and right earphones, and patient response switch corresponding to the master pane. System will come equipped with the equipment needed to perform a full comprehensive hearing test. This includes a GN Otometrics Madsen Astera audiometer (or equal), GN Otometrics OTOflex Tympanometer (or equal), Windows 7 GN Otometrics Touch Screen Monitor Kit (or equal), speaker, GN Otometrics Windows 7 Desktop (or equal), monitor, printer, Maico Ero-Scan Pro Combo (or equal) and printer system, Tremetrics Oscar Electro Acoustic Ear (or equal). Training will be conducted by the vendor. Delivery/installation/training is needed as soon as possible. The audiometer must be compatible with AUDBASE software and able to perform SPRINT test. The window for the exam booth must be at least 20"W x 48" L and the window between the exam booth wall and the control room must be at least 24" W x 48" L. This for observation of the patient and for patients who may be claustrophobic. QUOTE REQUIREMENTS: Quotes should include delivery date, discount terms and DUNS number. Quotes must also be submitted on an all or none basis and contain product specifications if quoting on an "equal" product. Specifications shall meet all criteria listed in the item description. The government will award a contract resulting from this combo synopsis/solicitation to the lowest priced, responsible offeror who meets the requirement.
