Description
The Department of Veteran Affairs, Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, SC 29209 has a requirement for a Fabricate and Install Storefront Type Interior Wall. The Government expects to award to a single contractor a contract which shall be Firm Fixed Price with a firm quantity and no option years. This requirement is being set-aside for Service Disabled Veteran Owned Small Business participation only. NAICS code for this requirement is 238390 with a Service Disabled Veteran Owned Small Business size of 500 employees. Only offers from those firms regularly established in the business of providing this type of equipment will be considered. Subcontracting will not be allowed. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. PERFORMANCE BASED WORK STATEMENT INSTALL STOREFRONT WALL AND DOOR 1. General: This performance based work statement describes the requirements for a non-personal service contract to provide and install storefront wall and lockable door in location indicated at the Department of Veteran's Affairs, Veterans Health Administration, William Jennings Bryan Dorn Medical Center (WJBDMC) in Columbia, South Carolina. 2. Background: a. The WJBD VA Medical Center is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. Annually, the medical center serves approximately 63,000 patients. Satellite outpatient clinics are located in Anderson, Greenville, Florence, Orangeburg, Rockhill, Sumter, and Spartanburg, South Carolina. 3. PERIOD OF PERFORMANCE: TBD 4. PLACE OF PERFORMANCE: Inside the main hospital, building 100, 1st floor in the Emergency Room department waiting area. 5. DAYS AND HOURS OF OPERATION: Monday - Friday, 8:00 a.m. to 4:30 p.m. excluding federal holidays. Evening and weekend work is expected due to congestion during the day. 6. PERFORMANCE REQUIREMENTS: a.The Contractor shall provide all labor, equipment, material, and supervision to successfully install storefront wall and door in location identified. Colors and glazing will be the same colors and finishes as already installed. b.The Contractor shall keep the work areas clean and neat and remove any debris regularly. c.The contractor shall maintain egress routes and not block exit hallways. d.The contractor shall retain control of their tools at all times and remain present while they are in use or are not secured. e.The contractor shall cut all materials at their fabrication shop. Chop saws and other cutting machines will not be used on site. Only drilling for anchoring will be permitted. f.Working after hours will be the norm since this is a crowded place during the day. g.The Contractor shall complete the work within 30 calendar days from the date the notice to proceed is received. h.The contractor shall provide traffic control when it is necessary to work in the hallways. i.The contractor shall ensure access to the Emergency Department is maintained at all times. j.The contractor shall ensure that extension cords, if used, do not cross the hallway or paths of pedestrian travel. All cords and power tools must have intact grounding prongs unless the tools are double insulated. Damaged cords are not allowed. 7. CONTRACT PERFORMANCE MONITORING: The government reserves the right to monitor services in accordance with quality assurance surveillance plan. 8. INVOICES: Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer's Technical Representative (COTR), and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714. Project: Fabricate and Install Storefront Type Interior Wall The lowest price technically acceptable source selection process will be used as basis of award. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for all non-cost/price factors. **ALL factors must be rated ACCEPTABLE to be considered for award. Non-Cost/Price Factors Technical approach: 1.Vendor shall provide documentary evidence detailing the use a minimum of 75% recycled content in all aluminum extrusions. 2.Vendor shall submit a finished acrylic polyurethane paint sample per the specifications in the statement of work. 3.Vendor shall submit manufacturing drawings showing you comply with the specifications and match existing (including LED unit specifications that comply with this solicitation). 4.The Environmental Graphic Designer assigned to the design of the exterior signage must have a significant experience with wayfinding design and signage design of large medical centers including other VA Hospitals. Significant experience refers to having designed wayfinding and signage systems for multiple large sized (250+ bed) medical centers as well as mental health hospitals, nursing homes and outpatient clinics. Should have at least 5 years of design, signage and artwork experience including membership in the Society of Environmental Graphics Designers and at least one person demonstrating NCIDIQ and LEED AP certification. Vendor shall provide documents and resume. 5.The vendor shall document three similar projects and discuss how the design team approached the process and developed the final package. This should include project photos. Additional company information relevant for evaluation in regards to vendor's design ability and approach may be provided. 6.Relevant Experience/Past Performance - The Relevant Experience/Past Performance evaluation will assess the relative risks associated with an offeror's likelihood of success in performing the solicitation's requirements as indicated by that offeror's record of Relevant Experience/Past Performance. Offerors must provide at least two (2) references and two (2) successfully completed contracts that demonstrate previous work experience in maintenance, inspection, and testing of the Electrical Power Distribution System and its components in healthcare, industrial, educational, and commercial facilities for a minimum of three (3) continuous years. Provide description of projects, dollar value, and Point of Contact(s) including phone/FAX numbers. Each factor will be rated as Acceptable or Unacceptable as defined below. Each proposal will be evaluated against the SOW and the stated evaluation criteria. Each proposal is evaluated using the same standards. "Lowest Price, Technically Acceptable" Evaluation Standards Acceptable ALL of the minimum acceptable criteria are clearly met by the proposal. The offeror's proposal meets the performance and technical capability requirements defined in the SOW. NOTE: Once the proposals have been determined to be "technically acceptable," award will based on cost/price only. Unacceptable Not all of the minimum acceptable criteria are met by the proposal. An unacceptable proposal contains one or more deficiencies. Proposal fails to meet specified minimum performance and technical capability requirements defined in the SOW. The following table will be used for each proposal received to score each LPTA factor as to whether it is Acceptable or Unacceptable. An organized site visit has been scheduled for 01/20/and 21/2015 at 10:00 AM EST. The site visit will be conducted at the DORN VAMC, Medical Center, 6439 Garners Ferry Road, Columbia SC 29209.
