- Amendment one **AMENDMENT ONE** Amendment one issued to provide a Question/answer. Submission date for proposals has been exetended to January 21, 2015. All other terms and conditions remain the same. [VENDORS MUST ACKNOWLEDGE AMENDMENT ONE] This is a combined synopsis/solicitation for a commercial items/services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Pursuant to procedures prescribed in Part 12, Acquisition of Commercial Items, and Part 13.5 Test Program for Certain Commercial Items. Solicitation Number RFP 15-239-SOL-00005 is being issued as a Request for proposal (RFP) and should be referenced on all correspondence. The RFP document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-78, effective 26, DEC, 2014. This solicitation is a total small business set aside. The NAICS code is 561320. The small business size standard is $13.5 million dollars. The Department of Health and Human Services/Indian Health Service has a requirement for Registered Radiology Technologist Services for the Bemidji Area Indian Health Service. Performance location is at the Red Lake Indian Hospital, located in Red Lake, MN. Please see attachment for additional information. POC: karen.mcdonald@ihs.gov. The Government contemplates award of a firm fixed-price, contract resulting from this solicitation. Base + 4 option years anticipated. Be advised that all interested parties must be registered in the System for Award Management (SAM) database. BASIS FOR AWARD The Government intends to make an award without discussion therefore; the initial proposal shall contain the offeror's best price and shall be determined to be the overall best value and most advantageous to the Government.
