Description

IAW VA solicitation VA248-15-Q-0074 the following has been amended and added 1. Driver License as reads: Class D driver's license Amend to read: Class E driver's license 2. As reads in the SOW: 13(I)(b) " the offeror shall show evidence of certification by the Florida Department of Business and Professional Regulation and Department of Highway Safety and Motor Vehicles." Amend to read: 13(I)(b) "the offeror shall show evidence of certification by the Department of Highway Safety and Motor Vehicles IAW with Florida State Statues and requirements for Non-Emergency medical transportation licensures and certifications." 3. Add: Past Performance Survey (attached) 4. Add: Questions and Answers (attached) 5. Add to SOW (Technical Criteria, 13(1)(b)): (clarifies "authorizations to transport in/out of required Counties designated for pickup/delivery") Provide copy of "Certification of Public Convenience and Necessity" required for transport in/out of all seven (7) counties; as required by Florida Department Health and Rehabilitative Services and Florida Statues for Non-Emergency Medical Transport. 6. Add to Solicitation: Request for Information period ends: 1/15/2015 at 0800.
