Description

REQUEST FOR SF 330s This is a request for Standard Form 330, not a request for proposal to provide professional Architectural/Engineering design services for project #556-305: New Parking Garage at the Captain James A. Lovell Federal Health Care Center (FHCC) in North Chicago, IL 60064. The Statement of Work (SOW) includes Schematic Design, Design Development, Construction Document, and Construction Period Services phases of design process, utilizing the provided programming/schematic design information for the study and design of building a new seven (7) level, eight hundred seventy-five (875) parking space garage. Provide any additional investigation and planning to complete the development of the site. All services related to the Statement of Work (SOW) shall meet local, state, federal, Department of Defense (DOD)/Department of Navy (DON), Veterans Affairs (VA), Veterans Integrated Service Network (VISN), Americans with Disabilities Act (ADA), and National Fire Protection Association (NFPA) Life safety codes. The A/E will develop standard construction documents for bidding and construction by a qualifying general contractor. The consulting A/E firm will also provide construction period services. In accordance with VAAR 819.7003(b), SDVOSB firms must be verified in the VIP database or hold a "Re-Verification" symbol listed next to the firm's name (this symbol indicates that the firm is undergoing the re-verification process at CVE), prior to submitting an SF 330 for this solicitation. All interested parties must be registered and current in the System for Award Management (SAM) database. If you are not registered in the System for Award Management (SAM) database, you may register online at https://www.sam.gov. This request for SF 330s is a 100% set-aside for Service-Disabled, Veteran-Owned Small Businesses. The architectural/engineering firm must be located within 200 mile radius of the Captain James A. Lovell Federal Health Care Center, 3001 Green Bay Road, North Chicago, IL 60064. The 200 mile radius will be determined using the following website: http://www.freemaptools.com/radius-around-point.htm. Prospective firms are reminded that per VAAR 852.219-10 and by its submission of an offer and execution of a contract, the Offeror/Contractor agrees that in performance of the contract in the case of a contract for services (except construction) that at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. Evaluation Factors Selection Criteria will be based on the following: 1) Previous experience in project type, complexity, scope and size similar to the solicited requirement. 2) Professional qualifications necessary for satisfactory performance of required services. 3) Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules. 4) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials. 5) Capacity to accomplish the work in the required time. 6) Location in the general geographical area of the project and knowledge of the locality of the project. Deliverables A/E firms, which meet the requirements listed in this announcement, are invited to submit two (2) original hard copies and one (1) electronic copy (one CD media or email (if under 7 MB), no flash drives) of the completed SF 330 no later than 3:00pm CST, Thursday, February 12, 2015.
