- Amendment 001 CSOSA-15-R-0140 Past Performance Questionnaire (PPQ) Word Format Amendment 001 to solictation provides responses to vendor questions, extends the date and time of closing, and administrative changes to the solictation. The Court Services and Offender Supervision Agency (CSOSA) has a requirement for an individual or firm with the capability to conduct executive leadership group facilitation, assessments and ongoing support. The contractor shall provide consultation to the Director and Executive Leadership Team consisting of a baseline climate assessment, the facilitation of off-site meetings for planning and development and ongoing executive coaching. The Agency anticipates a single Firm Fixed Price (FFP) award. Please read the solicitation for the statement of work and instructions for submitting an offer to the agency.
