- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Property and Fiscal Office, 9 National Guard Road, Columbia, South Carolina intends to award a firm fixed priced contract for the following Distinctive Unit Insignia (DUI) Metal Heraldic Insignia (unit crest). Item 0001 51st Military Police Battalion Unit Crestqty 1,600ea (Specifications for crest attached) Delivery Date: 30 days After First Article Testing approved by TIOH Item 0002 Die Casting Feesqty 1ea Item 0003 Discounts to Governmentqty 1ea Special Instructions: a. All insignia must be manufactured by firms certified by the U.S. Army Institute of Heraldry (TIOH) in accordance with Army Regulation (AR) 672-8. b. Authority to Manufacture (AR 672-8). Upon award, the manufacturer is required to submit preproduction samples of all insignia to TIOH. Once samples are approved, TIOH will issue a letter to the manufacturer authorizing the production of the insignia item. A copy of this letter will be forwarded by the vendor to USPFO for South Carolina prior to the manufacturing of the items. c. DUI must have the hallmark of the manufacturer stamped on the reverse side of the insignia item. In addition, DUI must have quote mark Made in USA quote mark stamped on the reverse side. d. Procurement of insignia from non-certified manufacturers or from foreign or offshore sources is not authorized. All quotes will include a statement that the insignia will be made in accordance within the specifications and tools provided by TIOH. Attachments included with this solicitation are: -TIOH Certified DUI Manufacturers List -TIOH 51st MP Bn DUI Specifications - Federal Acquisition Regulation (FAR) and Defense Federal Acquisition Regulation (DFARS) provisions. Solicitation, W912QG-15-T-0004 is issued as a request for quotes (RFQ) in accordance with FAR Parts 12 & 13. This procurement is a 100% small business set aside. The NAICS code for this acquisition is 332999 and the size standard is 750 employees. To be considered for award, the offeror must be actively registered in System for Award Management (SAM), which can be accessed as follows: https://www.sam.gov/portal/public/SAM/. All vendors MUST have a publicly visible registration in SAM. Offerors must include with their offer, the following information: Duns Number, CAGE Code Number, and Federal Tax ID Number. All Requests for Information (RFIs) (questions) shall be sent to blake.s.cromer.civ@mail.mil no later than 1000 (10:00 AM ET) Wednesday, 21 January, 2015. Reference the solicitation number, W912QG-15-T-0004 in the subject line when requesting information. Responses (answers) to all RFIs (questions) will be provided no later than 1500 (03:00 PM ET) Wednesday, 21 January, 2015.
