Description
STATEMENT OF WORK (Supplies) GREEN TISSUE PROGRAM 1.GENERAL: The contractor shall provide a Green Tissue Program at the Department of Veterans Affairs (VA) locations listed in paragraph 4 below and in accordance with performance requirements in paragraph 6 below. This contract will not be based on price alone. The criteria will be based quality, recycling requirements, waste minimization and cost. 2.BACKGROUND: The Jack C. Montgomery VA Medical Center (JCMVAMC) is a Joint Commission accredited, complexity Level 2 facility serving Veterans in 25 counties of eastern Oklahoma. The Medical Center is comprised of a hospital, which provides primary and secondary levels of inpatient medical and surgical care, and three outpatient clinics that provide primary and consultative care in medicine, surgery, and mental health. As part of the South Central VA Health Care Network, the hospital and clinics have ready access to nine sister facilities for referral and use the VAMCs in Oklahoma City and Little Rock for the majority of tertiary services. JCMVAMC provides services for more than 45,000 enrolled veterans in four locations. In Muskogee, the medical center operates 110 hospital beds and which includes an Inpatient Behavioral Health Unit and an Inpatient Rehabilitation Unit. JCMVAMC also operates VA-staffed Community Based Outpatient Clinics (CBOC) in Tulsa, Hartshorne and Vinita. Tulsa's Behavioral Medicine Service moved to a new location in January 2006 to accommodate the increased need and therapy rooms. 3. PERIOD OF PERFORMANCE: Interim time will be a part of the phase in and phase out process and will also carry the facilities through to the base year. One Base year with the possibility of four one year option periods: Base Year: February 2015 to September 30, 2015 1st Option PeriodOctober 1, 2015 to September 30, 2016 2nd Option PeriodOctober 1, 2016 to September 30, 2017 3rd Option PeriodOctober 1, 2017 to September 30, 2018 4th Option Period October 1, 2018 to September 30, 2019 4. PLACES OF PERFORMANCE: a.Jack C. Montgomery VAMC 1011 Honor Heights Drive Muskogee, OK 74401 b.Ernest Childers VA Outpatient Clinic (Tulsa) 9322 East 41st St Tulsa, OK 74145 c.Vinita Outpatient Clinic 269 S 7th St Vinita, OK 74301 5. DAYS AND HOURS OF OPERATION: Monday - Friday, 8:00 a.m. to 4:30 p.m. excluding federal holidays. 6. DEFINITIONS: a. Recovered paper material: paper waste generated after the completion of a paper making process, such as post-consumer materials, envelope cuttings, bindery trimmings, printing waste, cuttings and other converting waste, butt rolls and mill wrappers, obsolete inventories, and rejected unused stock. Recovered material, however, shall not include fibrous waste generated during the manufacturing process, such as fibers recovered from wastewater or trimmings of paper machine rolls (virgin mill broke), regardless of whether such materials are used by the same or another company and shall also not include fibrous by-products of harvesting, extractive or woodcutting processes or forest residues such as bark. b. Post-consumer material: those finished products, packages or materials generated by a business or consumer that have served their intended end uses, and that have been recovered from or otherwise diverted from the waste stream for the purpose of recycling. c. Tissue: Tissue for the purpose of this Statement of Work (SOW) includes toilet paper, hand tissues, bi-fold hand tissues, feminine hygiene products, baby changing station covers and toilet covers. The toilet paper and hand tissues are two types. d. Green Tissue Program: Includes the making and process of tissue products. This includes the dispensing devises and by products associated with the packaging and distribution. Manually operated dispensers preferred. e. Green Seal: Green Seal is a non-profit organization devoted to environmental standard setting, product certification, and public education. Green Seal's mission is to work towards environmental sustainability by identifying and promoting environmentally responsible products, purchasing, and production. Must meet or exceed Executive Order 13148, Greening the Government through leadership in Environmental Management and VHA Directive 2005-011, Green Environmental Management System (GEMS). f. Absorbency Rate: Water absorbency is a measure of the amount of water absorbed by the wetted surface of paper and board materials. This test determines the liquid sorption rate of bibulous paper, paperboard, and paper products using gravimetric principles. A set volume of water is dropped onto the paper surface and the time for the drop to be absorbed is measured. The water absorbency rate is used as a measure of the acceptability of tissue, toweling and blotter papers in sportive tasks. 7. PERFORMANCE REQUIREMENTS: Contractor shall ensure all products must be made in accordance with reasonable industry practice with respect to holes, tears, wrinkles, cleanliness, absorbency rates, foreign materials or dirt. It must have no disagreeable odor, either wet or dry, in accordance with reasonable industry practice. Edges of the product must be cleanly cut and not ragged. Product must dispense properly from the box or fixture. Contractor shall supply all dispensing units for Green Tissue items. Where feasible provide the environmentally friendly dispensing units. a. Toilet Tissue: The contractor shall provide each roll of bathroom tissue which must contain at least 40 square feet of product (equivalent to approximately 300 x 4.5 x 4.4 inch sheets). Each box of facial tissue must contain at least 70 square feet of product (equivalent to approximately 175 x 8.0 x 8.0 inch sheets). The fiber in bathroom tissue shall contain 100% recovered materials, including 20% post-consumer materials. b. Paper Towels: The contractor shall ensure fiber in paper towels and napkins contain 100% recovered materials and at least 40% post-consumer material by weight. The percentage of recovered material and post-consumer material shall be calculated and certified based on the FIBER weight of the paper. Calculations will also be performed based on the total weight of the paper. The recovered material used in making a certified product shall not be bleached using chlorine or any of its derivatives (such as hypochlorite and chlorine dioxide). Provide a version of paper towels for evaluation. c. Feminine Hygiene Products: The contractor shall ensure Tampons and panty liners are made from organic cotton (no irritating, synthetic materials here), whitened without the use of chlorine bleach, compact, and safe for sensitive skin. Feminine Hygiene Products may be quoted as Open Market. d. Toilet Seat Covers: Contractor shall ensure100 % Biodegradable seat covers provide a hygienic barrier for sanitary protection. e. Baby Changing Station Replaceable Covers: Contractor shall ensure 100 % Biodegradable covers provide a hygienic barrier for sanitary protection while changing diapers. f. Bleaching: Chlorine and its derivatives shall not be used. g. Post-consumer Content: the post-consumer content of a product shall be determined by measuring the average product fiber utilization over a period of no longer than three months. h. De-inking of Recovered Paper: recovered paper shall not be de-inked using a solvent containing chlorine, or any chemicals listed by the EPA under Section 313 of the Emergency Planning and Community Right To Know Act. i. Additional Ingredients: the product (not including packaging) shall not contain any added pigments, inks, dyes, or fragrances. j. Dispensing Units: Contractor shall supply all tissue dispensing units at no cost to the government. Contractor will be responsible for additional units as construction and renovation projects are completed. All dispensers must offer key ring sets for popular paper towel and toilet tissue dispenser brands or universal key system. 8. PACKING REQUIREMENTS: a. The core of a roll of bathroom tissue or the box used to package facial tissue must be made from 100% recovered fiber, or coreless. b. A consumer package of bathroom tissue must contain at least 4 rolls unless the outer wrapper has been manufactured from 100% recycled materials. c. Packaging shall not contain inks, dyes, pigments, stabilizers, or any other additives to which any lead, cadmium, or hexavalent chromium has been intentionally introduced. d. The sum of the concentration levels of lead, cadmium, mercury, and hexavalent chromium present in any package or packaging components shall not exceed 100 parts per million by weight. 9. PHASE IN/PHASEOUT: At the time the contractor is given the notice to proceed there will be a four week phase in phase out overlap period. This time frame will be allowed for the existing contractor(s) to remove their dispensers and the new contractor for placement. 10. REFERENCE: a. Executive Order 13148, Greening the Government through Leadership in Environmental Management. b. VHA Directive 2005-011, Green Environmental Management System (GEMS) c. International Organization for Standards (ISO) 14001 12. ADMINISTRATION a. Facility Orientation: An initial orientation of the facilities shall be conducted by the COR at the start of the contract. The contractor shall be responsible for conducting orientation for new employees thereafter. b. Damage to Government Property: Contractor is responsible for all damaged property. This includes holes in walls, tile replacement and damaged grout lines. c. Installation Guidelines: All dispensing devises shall be installed will all current VA Construction Guidelines. (i.e. Architectural Barriers Act, ADA, and UFAS) d. Accident Reporting: In the event an accident occurs on the Department of Veterans Affairs property or involving Government personnel or property, the contractor shall contact the VA Police immediately. A report shall be provided to the Contracting Officer and COR in writing that shall include the following: (1) the time and date of occurrence; (2) the place of occurrence; (3) a list of personnel directly involved; and (4) a narrative or description of the accident to include chronological order of the accident and circumstances; (5) corrective action to prevent future occurrences. c. Locations: The government reserves the right to modify the contract as needed to add or delete VA Locations. d. Federal Holidays: New Year's Martin Luther King Jr. Day President's Day Memorial Day Fourth of July Labor Day Columbus Day Veteran's Day Thanksgiving Christmas e. Invoice: Payment will be made upon receipt of a properly prepared, itemized invoice, validated by the COR, and submitted to VA FSC, P. O. BOX 149971, AUSTIN, TX 78714. (1) A properly prepared invoice will contain: oInvoice Number and Date oContractor's Name and Address oAccurate Purchase Order Number oOne Monthly Billing Cycle oTotal amount due oQuantity Billed oPrice per item billed (2) A separate invoice will be prepared for each facility.
