Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6-streamlined procedures for commercial items and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit bids. Solicitation VA-249-15-Q-0247 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, as written solicitation will not be issued. This solicitation is issued as a Total Small Business Set-aside under NAICS Code 621511 and the small business size standard $32.5 Million. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and possess the skills and capability necessary to provide all labor, transportation expenses, expertise, and supervision required for laboratory services, to include pickup and transport of specimens per the attached Statement of Work (SOW). This service is required for the VA Medical Center, 800 Zorn Avenue, Louisville, Kentucky, 40206 in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on the items as provided below.
