Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-15-Q-0149 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 339112 and the business size standard is 500 employees. Requestors shall state their business size on their proposal. Proposed items must be BRAND NAME OR EQUAL to the items requested below. They must perform to exact specifications of the brand names referenced. Contractor may be asked to prove the performance and characteristics through schematics or demonstration, to be scheduled with the Contracting Officer. All responsible sources may submit a proposal which shall be considered by the contracting officer. Number: 1 GRAND TOTAL TENTATIVE ORDER FOR REQUIRED SUBSTITUTE SUPPLIES TO SUSTAIN PATIENT CARE FOR HEMODIALYSIS THROUGH RENOVATION COMPLETION. THIS PO IS BEING PUT IN PLACE AS REINFORCED METHOD TO ENSURE SUPPLIES ARE AVAILABLE ON A CONTINUAL BASIS. TENTATIVE DELIVERIES SHOULD BE SCHEDULED TO DELIVER 2 PALLETS OF EACH ITEM (60 CASES PER PALLET) FOR A DELIVERY DATE OF MONDAYS. THIS SHOULD BE ESTABLISHED TO BE THE NORMAL DELIVERY FOR THIS ORDER UNLESS OTHERWISE AGREED TO MODIFIED CHANGES BY DETROIT VA. IT SHOULD BE ALSO BE UNDERSTOOD THAT IF RENOVATION IS COMPLETED BEFORE ALL ITEMS ARE CONSUMED THE VA WILL INFORM VENDOR WITHOUT PENALIZATION OF THE REMAINING ORDER. Items shall be delivered to Richard L. Roudebush VAMC, 1481 W. 1oth Street, Indianapolis, IN 46202 The provision at 52.212-1, Instructions to Offerors-Commercial applies to this acquisition and offerors must comply with all instructions contained within it. Award will be made to the lowest price, technically acceptable offer. The following additional provisions and clauses apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4, 6, 23, 26, 27, 28, 29, 31, 38, 42, and 48..
