- Description
- (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The applicable solicitation number is VA250-15-Q-0114 and it is issued as a request for quote (RFQ). Ensure responses include this solicitation number. (3) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-78. (4) This procurement is a 100% Severely Disabled Veteran Owned Small Business (SDVOSB) set-aside IAW FAR 19.502-2(a), 38 U.S.C. 8127; VAAR 852.215-70 and VAAR 852.215-71. The North American Industry Classification System (NAICS) code for this acquisition is 611699, All Other Miscellaneous Schools and Instruction. For the purpose of this procurement the small business size standard is $11.0M. (5) CONTRACT LINE ITEMS: The cost for the following Contract Line Item Number (CLIN) will be submitted as follows and will include all associated costs to perform services in accordance with Para (6), Requirements and the attached Statement of Work: CLINQUANTITYUNIT OF ISSUEUNIT PRICEEXTENDED PRICE Two (2) Day / Sixteen (16) Hours Customized Basic Nursing Moulage Course for Ten (10) Students (Includes all learning materials required) 16 Hr $ $ Two (2) Instructors for Two (2) Days of Instruction; Including Air and Ground Transportation, Lodging and Meals for Four (4) days (IAW JTR) http://www.defensetravel.dod.mil/site/travelreg.cfm 1 Ea $ $ Shipping Costs of all training Materials and Instructor Equipment 1 Lot $ $ (6) REQUIREMENTS: This is a requirement to perform Customized Basic Nursing Moulage Training for Ten (10) medical providers. Training will be conducted in accordance with the attached Statement of Work. (7) PERIOD OF PERFORMANCE: Anticipated schedule within 30 days after award; two workdays in February 2015. The Two Day Training must be completed by 30 September 2015. (8) The following clauses and provisions apply to this solicitation (available from http://www.acquisition.gov/far/): FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this solicitation. No addenda are attached. FAR 52.212-2, Evaluation -Commercial Items-Lowest Price Technically Acceptable (LPTA) Evaluation of technical capability: to be considered technically acceptable, offerors must provide a proposal detailing what methods will be utilized to meet the Government requirement. At a minimum this shall include: Personnel to be utilized; experience in performing similar testing, Required Certifications, and Offeror's Technical Qualifications. Failure to supply the minimum items may result in the submitted offer being deemed non-responsive. The Government intends to award without discussions so offeror's should propose their best terms during initial proposal. The Government reserves the right to open discussions if no offer submitted meets all the required specifications. Evaluation of Past Performance: Provide a list of three (3) projects completed during the last three (3) years that is similar in scope to this requirement and a point of contact's name, telephone number, and e-mail address. Evaluation of Socioeconomic Considerations: In accordance with, 38 U.S.C. 8127; VAAR 852.215-70 and VAAR 852.215-71, the Government shall evaluate the offeror's SDVOSB and/or VOSB status within the VA Vendor Information Page (www.vip.vetbiz.gov) and provide credits to the submitted offer in evaluation of price. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Background. Clinical simulation training is used to train providers: physicians, nurses, residents, etc. We currently performing this work through grant funds, "Enhancing Provider Competencies in VISN 10", and "Palliative Care", funded by the Office of Rural Health. This funding allows us to travel throughout the state of Ohio to train providers, in addition training providers at the Dayton VA Medical Center in the Simulation Laboratory. This effort is necessary to facilitate and enhance our provider's competencies. Activities sometimes include high-fidelity simulations with male and female mannequins and/or standardized patients. To ensure that our learners have exposure to simulated diseases and symptoms we would like to provide a moulage training course to our educators in the simulation center. We are seeking an instructor that has a respective reputation and extensive experience in moulage training. We also ask that instructor be well published in this area of expertise in the form journal articles and/ or books. 2. Scope. Medical moulage helps with simulated illnesses or wounds on a manikin in order to better train healthcare professionals. During this two-day moulage training workshop, learners will be guided to learn how apply the appropriate tools to simulate and mimic a number of diseases and conditions: peripheral vascular disease, venous insufficiency, extremity skin ulcerations secondary to PVD and diabetes, body fluids, drainage, secretions, feces, vomit, abscesses, infiltrated IV, subcutaneous emphysema, hematoma, scars, scrapes and excoriations, edema, pitting and non-pitting, cyanosis, surgical wounds, infected and non-infected, radiation injuries, and signs and injuries secondary to weapons of mass destruction. 3. Specific Tasks. Not applicable. 4. Performance Monitoring Not applicable. 5. Security Requirements This contract does not require any participants to have access to patient records, data or VA computer systems. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No GFE or GFI will be issued. 7. Other Pertinent Information or Special Considerations. There is no pertinent information or special considerations for this contract due to the fact all participants under contract will be performing a simulated case scenario. No follow-up on work is required and no potential conflicts of interest have been identified. Likewise there is not identification of non-disclosure requirements since no sensitive material will be involved. 8. Risk Control Each educational encounter is a training session. All infection and safety control standards will be met and adhered to as if in an actual hospital setting. 9. Place of Performance The place of performance will be the simulation center at the Dayton VA Medical Center Facility. Simulation training at the Dayton VA Medical Center will be held in the Simulation Laboratory in Building 315. 10. Period of Performance Training should be conducted within 30 days of contract award. 11. Delivery Schedule.
