Description

This solicitation is a "SOURCES SOUGHT" only. This is not a request for formal proposals or quotes. No formal solicitation document exists at this time. This request for information (RFI) is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this solicitation will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing a full maintenance contract with a firm fixed-price with a Base Period of April 1 through September 30, 2015 with two possible Option Years for the Aqua Pure water purification system in the Hemodialysis unit at the Albany VA Medical Center. The system shall be maintained in accordance with manufacturer's recommendations and frequency. It is also the intent to have the vendor be on-call for emergency service requests outside normal business hours, nights, holidays, and weekends during critical times when needed. All work shall be performed in strict accordance with the manufacturer's standards (AmeriWater, 1257 Stanley Ave., Dayton OH 45404). Only vendors certified by AmeriWater licensed vendors and maintenance technicians for AmeriWater products will be allowed to perform any maintenance function of this contract. Evidence of this certification must be provided before any work may commence. Interested parties must furnish supporting evidence in sufficient detail to demonstrate the ability to provide the requirements shown above including past performance experience on similar AmeriWater products. Equipment List: Ameriwater model MRO2X 10 Station Hemodialysis RO Water Treatment System s/n 26035, VA EE #185472 System located in room D232, Hemodialysis. (2) Ameriwater model MROS RO+ portable RO units (VA EE #153514 and 153515). (14) B Braun model 710500L Dialog + Hemodialysis Machines (VA EE #163725, 163727, 163728, 163729, 163730, 163731, 163732, 163733, 163734, 164251, 164252, 164253, 164254, 164255). Place of Performance: Department of Veterans Affairs Stratton Albany VA Medical Center, 113 Holland Avenue, Albany, NY 12208-3410. Response: Responses to this Sources Sought Notice should be able to demonstrate the firm's ability, capability, and responsibility to deliver the service listed above. Responses should include the following information: Business name, address, DUNS number, business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, Small Business, Large Business, etc.), and point of contact to include e-mail address and telephone number. All offerors must be registered in the System for Award Management (www.sam.com), and Representations and Certifications must be completed prior to any future award. Any offeror capable of providing this service shall notify the Contracting Officer by email within 14 calendar days from the date of the publication of this synopsis. Determination by the Government not to compete this proposed contract action based on responses to this notice is solely within of the discretion Government.
