- INTRODUCTION. The Connecticut Army National Guard Occupational Health Department requires medical services to conduct medical surveillance examinations on Connecticut Army National Guard employees. These examinations will be for the federal workforce and is not applicable under this contract to State Employees. SCOPE. The selected Physician shall provide qualified Health Care Providers (HCPs) to support the Occupational Health Department of the Connecticut Army National Guard to conduct baseline medical exams as well as follow up exams as needed upon the hiring and request of the employee or the Occupational Health Department. Medical exams are also required on exposures during the course of employment to hazardous materials. There is not a set amount of patients that will be seen during the duration and life of this contract. The number of patients is dependent on the number of new hires in the fiscal year, budgeting possible deployments and sequestration. All services will be performed at the selected Physicians Offices. Occupational Health Department is the only agency to request CALLs against this requirement are Ordering Activities (OAs). OAs may issue CALLs IAW the terms and conditions of the basic contract and will detail the OA's specific requirements. Physicians are defined as individuals possessing a degree in medicine or osteopathy and licensed by a State, Commonwealth, territory, or jurisdiction in which to practice medicine. The Physician awarded this contract must be licensed and credentialed in the State of Connecticut. BACKGROUND. The mission of this medical monitoring program is to ensure the CTARNG has available at all times a healthy military/civilian force supported by a health care system. This program is to ensure that the force also stays healthy and when required is able to see a physician as the need requires.
