- attachment 3 request for clarification attachment 4 fy15dockside contimuation of the combined synopsis/solicitation attachment 5 GFP report form Attachment 1 specification AlexHaley attachment 2 schedule of prices This is a combined synopsis/solicitation HSCG85-15-R-P45201 for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is total a small business set-aside procurement. The NAICS is 336611 with a size of 1000 employees. The contract will be awarded on a firm, fixed-price basis. Only one award will be made to the responsive offeror under this solicitation. To receive an award, the Contractor must be registered in the System for Award Management (SAM) www.sam.gov. This requirement is for dockside repairs aboard USCGC ALEX HALEY (WMEC-39) IAW the attached specification (ATTACHMENT 1). Performance will commence on 18 May 2015 and complete no later than 27 July 2015 at the Cutter's home moorage in Kodiak, AK.. QUESTIONS AND REQUEST FOR DRAWINGS: Drawings will be made available upon request by email to the contract specialist, Gerald Acock at Gerald.r.acock@uscg.mil. Requests for clarifications are due no later than 13 February 2015 at 10:00 AM Hawaii Standard Time. See ATTACHMENT 3, Request for Clarification. After this date and time, further requests may not be accepted due to time constraints. The Contractor shall furnish all the necessary labor, material services, equipment, supplies, power, accessories, and such other things as are necessary, except as otherwise specified, to perform dockside repairs to the vessel, in accordance with the solicitation and attached specifications. Failure to read the specifications in their entirety will not relieve the offeror from the responsibility for properly estimating the difficulty or price of successfully performing the work. Failure to do so will be at the offerors risk. All proposals submitted in response to this solicitation shall stand firm for 90 calendar days from the response date of this solicitation. INFORMATION REQUIRED WITH PROPOSAL SUBMISSION: 1. DUNS number 2. Small Business Certification 3. Welding Certifications and Procedure 4. Proof of Insurance 5. Any issued amendments to the solicitation 6. Attachment 2 - Schedule of Items - Prices. Please complete CLIN 0001 through CLIN 0003. Contract Line Item Numbers (CLINS) are marked as "D" for Definite, Optional CLINS are marked "O". 7. Attachment 3, Continuation of Combined Synopsis/Solicitation. Review each page of this attachment thoroughly as it contains more proposal instructions, clauses and provisions which may contain blocks that must be completed. Offerors that elect to use commercial delivery services for delivery of contract proposals are advised that such delivery must be made to 400 Sand Island Parkway, C4IT/NED Building, Honolulu, HI 96819 by the delivery time and date cited below to be considered timely. MAILED and E-MAILED offers will be accepted. FAXED OFFER will NOT be accepted. E-MAILED proposals will be sent to: Gerald.r.acock@uscg.mil. Please note, proposals that do not include all required documents will be considered incomplete and will not be considered in the evaluation process of the requirement. PROPOSAL DUE DATE: Proposals must be received not later than 26 February 2015, 10:00 AM Hawaii Standard Time. Proposals received after the date and time specified will not be considered. ANTICIPATED AWARD DATE: The contract is expected to be awarded on or about 20 March 2015. Work covered by this solicitation shall be completed within 70 calendar days from the date the vessel becomes available to the Contractor (May 18, 2015 through July 27, 2015). An arrival conference will normally be held on the day of the scheduled start date. PLACE OF PERFORMANCE: All work will be accomplished onboard the cutter at BASE KODIAK, AK.
