Description
COMBINED SYNOPSIS/SOLICITATION PRICE/COST SCHEDULE STATEMENT OF WORK Notice This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number is HSFELA-15-R-0001 and is issued as a Request for Proposal (RFP), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-43. The associated North American Industrial Classification System (NAICS) code for this procurement is 561210, Facilities Support Services, with a small business size standard of $35.5 million. This acquisition is 100% set aside for small business concerns that reside or primarily conduct business in the State of Louisiana. Offeror(s) shall furnish documentation to support its representation that it resides or primarily conducts business in the State of Louisiana. Refer to FAR 52.226-3 for factors considered. Offers shall not be accepted or evaluated from offerors who are not Louisiana small businesses in accordance with FAR 52.219-6. Offers shall not be accepted or evaluated from offerors who are not local Louisiana businesses in accordance with FAR subpart 26.2. Description of Requirement The Government's intent is to issue a firm fixed priced contract from this solicitation to support ground maintenance services at the FEMA Baton Rouge Temporary Staging Site (BRTSS), 2695 N. Sherwood Forest Boulevard, Baton Rouge, Louisiana 70815. The period of performance for this contract shall be from March 1, 2015 through September 30, 2015 with three (3), six (6) month option periods. The anticipated award date is February 24, 2015. See attached Performance Work Statement (PWS), Attachment A, for specific performance requirements and tasks. Site Visit A site visit will be held on Tuesday, February 3, 2015 from 10:00 am to 12:00 pm at the FEMA Baton Rouge Temporary Staging Site (BRTSS), 2695 N. Sherwood Forest Boulevard, Baton Rouge, Louisiana 70815. Please submit your name and contact information to the Contract Specialist at neddra.tucker@fema.dhs.gov by Thursday, January 29, 2015, 12:00 pm to confirm attendance and to receive access to the site. Proposal Submission Instructions Responses to this Request for Proposal (RFP) are due no later than 12:00 pm on Tuesday, February 10, 2015. Questions regarding this solicitation shall be submitted to the Contract Specialist no later than 1:00 pm on Wednesday, February 4, 2015. Questions received after this time and date may not be considered. Each Offeror is requested to submit an electronic version of your business's technical and price proposal. Offerors shall submit a technical proposal, Volume I, and a price proposal, Volume II. The technical portion of the proposal must be identified by a separate cover sheet from the price portion of the proposal with a cover sheet but BOTH shall be submitted for evaluation. The electronic version of your proposal shall be submitted to the Contract Specialist, Neddra P. Tucker at Neddra.tucker@fema.dhs.gov and Contracting Officer, Debra Boe, at debra.boe@fema.dhs.gov. (preferably in PDF format) Please include your DUNS Number, Tax Identification, Small Business Status and Authorized Representative contact information with your technical proposal submission. Basis for Award The Government's source evaluation will be based on a lowest price technically acceptable (LPTA) process. Accordingly, award will be made on the basis of the lowest evaluated price of the proposal meeting or exceeding the acceptability standards for non-cost factors, technical approach and quality control plan. This process does not permit tradeoffs. The following evaluation factors/criteria shall be evaluated by the Government: Volume I- Technical (non-cost) Factors- The technical factors are all of equal importance. Factor 1- Technical Approach Factor 2- Quality Control Volume II- Cost or Price Factors Factor - Price TECHNICAL FACTORS FACTOR 1: TECHNICAL APPROACH The Offeror's technical approach shall demonstrate an understanding of the requirements and provide the techniques and procedures that will be used to satisfy the requirements in a timely and cost effective manner. The Offeror's technical approach will be evaluated by assessing the likelihood that the proposed technical approach will meet the Government's requirements, including any associated risk of the Offeror's non-performance in the technical approach. This factor will be used to evaluate the degree to which the Offeror's proposed technical approach meets all requirements. Offerors shall demonstrate their understanding of the requirements stated in the PWS. Technical approach will be evaluated based on the extent to which the Offeror concisely and accurately discusses the nature of the services being requested as measured by the following which are considered of equal importance: • Understanding of the technical requirements identified in the PWS. • Detailed approach for providing for the technical requirements identified in the PWS. • A demonstration of the technical approach to be used to ensure the work is being performed on-time and properly. FACTOR 2: QUALITY CONTROL PLAN. The Offeror shall provide a Quality Control Plan (QCP) which clearly states the proposed methods and techniques that will be used to maintain effectiveness of the Offeror's quality control system through the use of their quality policies, quality objectives and analysis of data, corrective action plans, preventative actions, and management review. The plan shall, at a minimum, discuss the Offeror's overall approach and procedures for evaluating each of the tasks identified in the PWS, communicating with the Government, resolving deficiencies, and identifying potential improvements. The Government will evaluate the extent to which the offeror clearly states the proposed methods and techniques that will be used to maintain effectiveness of the Offeror's quality control system through the use of their quality policies, quality objectives and analysis of data, corrective action plans, preventative actions, and management review. COST OR PRICE FACTORS FACTOR: PRICE Offeror shall provide fixed unit prices for each CLIN for the basic requirement and option periods identified in Attachment B, Price/Cost Schedule. The Government will evaluate offers for award purposes by adding the total price for the options to the total price for the basic requirement. Evaluation of options does not obligate the Government to exercise the option(s). The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. The Government will evaluate the price proposal to determine reasonableness of price offered. Wage Determination Number 2005-2231 Revision No. 16 dated 12/22/2014 for East Baton Rouge Parish shall be applicable to the resulting contract/purchase order. See Attachment B, Price/Cost Schedule, and Attachment C, Wage Determination, to submit with your price proposal, Volume II. Solicitation Provisions and Contract Clauses The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.212-4, Contract Terms and Conditions - Commercial Item applies to this acquisition. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, shall apply: 52.219.6, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-9, 52.225-1, 52.225-13, 52.232-33. The contractor shall provide information necessary for the federal agency to comply with the 1) Emergency planning and toxic release reporting requirements tin EPCRA, PPA and E.O. 13148; and 2) Toxic chemical, priority chemical, and hazardous substance release and use reduction goals of section 502 and 503 of Executive Order 13148; and 3) Requirements for EMS and FCAs if the place of performance is at a federal facility designated by the agency. This information shall be provided upon contract award. In the addition, the following Homeland Security Acquisition Regulations (HSAR) shall be applicable to the contract: 3052.204.71; 3052.209-70; 3052.215-70; 3052.228-70; 3052.242-71; 3052.242-72; 3052.245-70. 52.219-6 Notice of Total Small Business Set-Aside. As prescribed in 19.508(c), insert the following clause: NOTICE OF TOTAL SMALL BUSINESS SET-ASIDE (JUNE 2003) (a) Definition. "Small business concern," as used in this clause, means a concern, including its affiliates that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the size standards in this solicitation. (b) General. (1) Offers are solicited only from small business concerns. Offers received from concerns that are not small business concerns shall be considered nonresponsive and will be rejected. (2) Any award resulting from this solicitation will be made to a small business concern. (c) Agreement. A small business concern submitting an offer in its own name shall furnish, in performing the contract, only end items manufactured or produced by small business concerns in the United States or its outlying areas. If this procurement is processed under simplified acquisition procedures and the total amount of this contract does not exceed $25,000, a small business concern may furnish the product of any domestic firm. This paragraph does not apply to construction or service contracts. 52.226-4 Notice of Disaster or Emergency Area Set-Aside. NOTICE OF DISASTER OR EMERGENCY AREA SET-ASIDE (NOV 2007) (a) Set-aside area. Offers are solicited only from businesses residing or primarily doing business in the State of Louisiana. Offers received from other businesses shall not be considered. (b) This set-aside is in addition to any small business set-aside contained in this contract. (End of clause) 52.233-2 Service of Protest. As prescribed in 33.106, insert the following provision: SERVICE OF PROTEST (SEPT 2006) (a) Protests, as defined in section 31.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the Government Accountability Office (GAO), shall be served on the Contracting Officer (addressed as follows) by obtaining written and dated acknowledgment of receipt from: FEMA Louisiana Recovery Office (LRO) Acquisitions 2nd floor Debra Boe Contracting Officer 1500 Main Street Baton Rouge, Louisiana 70802 (b) The copy of any protest shall be received in the office designated above within one day of filing a protest with the GAO.
