THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential vendors capable of providing services. The VA Network Contracting Office (NCO) 19, Contracting Office located at 4100 E Mississippi Ave, Suite 900, Glendale CO 80246 is seeking a potential qualified contractor to provide maintenance and service to Medivator Advantage Plus Automated Endoscope Preprocessors located at the George E. Wahlen Department of Veterans Affairs Medical Center, 500 Foothill Drive, Salt Lake City, Utah, 84148. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. The designated NAICS Code is 811219, Other Electronic and Precision Equipment Maintenance and Repair, size standard $20.5 million. Your response shall reference Sources Sought number VA259-15-R-0182. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of business, e.g. Services Disabled Veteran Owned small business, Veteran-owned small business, 8(a), HUBZone, Women Owned small business, Small disadvantaged business, or small business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The contractor shall provide full maintenance both remotely and on site, to include all replacement parts, labor, travel expenses, telephone support, tools, test equipment and other incidentals necessary to maintain and/or restore Advantage Plus Auotomated Endoscope Preprocessors and Veriscan LT scope Disinfection Units to OEM operating specifications in accordance with the manufacturer's recommendations. The general limit of the specified work area will be located in the VAMC, 500 Foothill Blvd, Salt Lake City, UT 84148, Bldg 1 & 14 GI area, OR area, 3- East. The maintenance shall be provided during the contract business hours of 8:00 AM-500PM, Monday thru Friday, excluding federal holidays. Medivator Endoscope Advantage Plus S/N 65345802 S/N 65345806 S/N 65345807 S/N 65345808 Veriscan LT3 Scope Disinfection Units S/N 201111000755 S/N 201100900693 S/N 201100900692 Preventive Maintenance 1.The contractor shall provide Gold Service Plan for the above identified Medivators. "Guaranteed Response Time: Field Service Engineers will use commercially reasonable efforts to respond to Customer's requests within 24 hours of notification to Customer Service Center. "Live Phone Support: phone support with Technical Support Engineers between 6:00 a.m.-6:00 p.m. (Central Time) Monday thru Friday. "Preventative Maintenance: Perform semi-annual System maintenance inspections to maintain factory specifications. "Parts Exchange: All parts required for the repair of the System (excludes instruments and accessories) are covered. "Labor & Travel Expense: Labor and travel conducted during normal business hours: Monday-Friday 8:00 a.m.- 5:00 p.m. Customer's local time, Certified Trained Technicians: The scheduled maintenance and service calls shall be performed by Intuitive trained personnel in accordance with the Agreement 2. All services for this contract must meet manufacturer's performance and technical specifications, Federal Regulations, and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and VA Regulations. Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System For Award Management (SAM located on the web at www.sam.gov). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail: to Yolanda.miller2@va.gov All information submissions should be received no later than 3:00 pm MST on Monday, February 2, 2015. This Sources Sought Notice is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this Sources Sought Notice will be treated as information only. The Government is not obligated to nor will it pay for or reimburse any direct or indirect costs associated with responding to this sources sought synopsis request. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. The information provided may be used by the Department of Veterans Affairs in developing its acquisition approach, statement of work/statement of objectives and performance specifications. Interested parties are responsible for adequately marking proprietary or competition sensitive information contained in their response. The Government does not intend to award a contract on the basis of this Sources Sought Notice or to otherwise pay for the information submitted in response to this Sources Sought Notice.
