- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Proposal LOI for a COMPETITIVE, firm fixed-price contract that will consist of one 12-month Base Year and four (4) 12-month Option Years for a total of five years.The Contractor shall conduct the work described in the Statement of Work (SOW ) dated 26 January 2015, independently; however, it is subject to inspection by the Project Manager under the Quality Surveillance Inspection Program (QASP). Enclosed you will find the Statement of Work (SOW ), a Question and Answer Form (Q&A), and Form SF1449 Solicitation/Contract/Order for Commercial Items. The award will be made based on the lowest priced proposal, which also meets or exceeds the acceptability standard for the non-cost evaluation factors. Price Evaluation will be based upon the Five Year Total proposed for the Base plus Four (4) Option Years. A. Non-Cost Evaluation Factors: The offorer MUST furnish the following non- cost evaluation factors: 1.Provide Offeror's names, address, point of contact, phone number, DUNS, CAGE CODE, and e-mail address. 2.Complete section 52.212-3 of the Solicitation - OFFEROR REPRESENTATIONS and CERTIFICATIONS -- COMMERCIAL ITEMS (DEC 2014) in the SF 1449 (Pages 7 thru 20) 3.Provide a Statement of Understanding that the awarded vendor must be registered with the System for Award Management (SAM) website at www.sam.gov. 4.Supply your main office's distance from Ukiah, California. 5.Provide Offeror's capability to execute this contract shall provide: a sample of a contract of this magnitude and complexity with demonstrated comparable scope, performed within the past 5 years - brief description of the project, customer name, timeliness of performance, customer satisfaction, dollar value of the project, location of service, and date of completion. 6.Provide at least two (2) example projects. 7.Offeror will attest that their company will comply with FAR Clause 52.219-14 (1); which states, quote mark Services (except construction). At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern. quote mark 8.Provide a detailed breakdown of how the company plans to fulfill the requirements as outlined in the Scope of Work. All submitted questions and answers will be provided to all offerors via Federal Business Opportunity website. C.
