- Chapter 9 RCS SF 1449 Solicitation Appraiser Questionaire This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is issued as, Request for Quotation (RFQ) DU205NC-15-Q-0022. (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC-05-78 (December 26, 2014). (iv) A notice regarding any set-aside and the associated NAICS code and small business size standard 531320 with a size standard of $7.5 million. (v) A list of contract line item number(s) and items, quantities and units of measure, 0001, Rent Comparability Studies, estimated quantities 45 each. (vi) Description of requirements for the items to be acquired (See attached Statement of Work). (vii) Date(s) and place(s) of delivery and acceptance and FOB point. The Date(s) are stated on page 3 (E. Deliverables and Schedule Requirements) of the Statement of Work. (viii) A statement that the provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition and a statement regarding any addenda to the provision. Proposals shall include all information required in this section which includes Schedule of Prices Offered, and a complete FAR 52.212-3. (ix) A statement regarding the applicability of the provision at 52.212-2, Evaluation - Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. (Factors of equal importance): Factor 1-Prior Experience-The offeror shall provide documentation of prior experience for a minimum of 3-year period prior to submission of the quotation, of the same or similar services relevant to the solicitation requirements, sufficient to ascertain the offeror would be able to perform the contract requirements with a high level of quality. FACTOR 2-Past Performance: Contractor shall provide a list of three (3) sources/references for completed work in the last three (3) years which are similar in scope, magnitude, relevance and complexity to this requirement. Please provide a POC, telephone number, Contract Number, brief scope of work, total dollar value, and period of performance. The Department of Housing and Urban Development may also contact other sources not listed. FACTOR 3-Licensing: Offer must provide a Certified General Appraiser License in the state of listed. FACTOR 4: Submit a redacted sample of Rent Comp Study or similar service provided by your company. FACTOR 5-Price. (x) A statement advising offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. All Quoter's must be registered in (SAM) System for Award Management, in order to perform work awarded by HUD. In accordance with Federal Acquisition Regulation 4.1103 (c) (1), failure for not being registered shall result in the withdrawal, of the successful offer, and will be awarded to the next otherwise successful offer able to comply with the registration requirement. (xi) A statement that the clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. (See Contract Clauses) (xii) A statement that the clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. (See Contract Clauses) (xiii) A statement regarding any additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices. (xiv) A statement regarding the Defense Priorities and Allocations System (DPAS) and assigned rating, if applicable. (N/A) (xv) The date, time and place offers are due. (RFQ-DU205-NC-15-Q-0022) with the following notation: MAILROOM: DO NOT OPEN. Mail quotes to. U.S. Department of Housing and Urban Development (HUD), 77 West Jackson Blvd, Chicago, Illinois 60604-3507, Room 2517. Any incomplete responses or responses received after the closing will not be accepted. Faxed or email quotation package will be accepted. The coversheet to the quotation shall have the offeror's information. The technical quotation shall only have the solicitation number. Quoted pricing shall be valid for 30 calendar days. Closing date for receipt of quotes is due on Wednesday, February 11, 2015, 14:30 Central Time.
