Description

The Department of Veterans Affairs (VA), Michael E. Debakey VA Hospital, Houston, Texas, intends to solicit proposal from contractors to provide Adult Day Health Care (ADHC). The VA anticipates issuing a solicitation for this requirement on or about February 5, 2015, with a closing date for proposals on or about February 20, 2015. The solicitation will be available for download from www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments. Michael E. Debakey VA Hospital is seeking businesses capable of providing all necessary personnel, equipment, and supplies associated with the full range of ADHC services to beneficiaries for whom such care is specifically authorized by the Veterans Affairs. It is understood that the type of patient to be cared for, newly diagnosed stroke and Alzheimer patients, under this contract will receive these services as a substitute for institutionalization. The purpose of providing ADHC is to enable functionally impaired individuals to reside in supportive home environments rather than in nursing home care facilities; to facilitate the return of older veterans to their home and coordinate long term care, thereby reducing re-admissions to hospitals and nursing homes; to maintain the older veteran at the highest level of functioning possible; to improve the quality of life for the participants among their pears; and to provide support and respite for the family and other caregivers to enable them to maintain veterans in the community. The care provided will include Physician service, nursing, social work services, occupational or physical therapy, assistance with activities of daily living, activity therapy, case management, nutrition service, including meals, and transportation. Contracts will have a base term of one year, with options for four additional one-year terms exercisable at the discretion of the Contracting Officer. North American Industry Classification System (NAICS) Code is 624120 and size standard is $27.5 Million and the solicitation will be issued Unrestricted/Open Market. All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in SAM (System for Award Management) database (information on registration requirements can be obtained by the Internet at https://www.sam.gov and submit their complete Annual Representations and Certifications (ORCA) via online at https://orca.bpn.gov at proposal due date/time.
