Description

The intention of the upcoming solicitation will be to provide Adult Day Health Care (ADHC) services to beneficiaries for whom such care is specifically authorized by the Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd, Houston, TX under NAICS Code 624120 with a business size of $11.0 for base period April 1, 2015 through March 31, 2016 plus four (4) option periods. The type of patient to be cared for under this agreement will receive these services as a substitute for institutionalization. The purposes of providing ADHC are: to enable functionally impaired individuals to reside in supportive home environments rather than in nursing home care facilities; to facilitate the return of older veterans to their home and coordinate long term care, thereby reducing re-admissions to hospitals and nursing homes; to maintain the older veteran at the highest level of functioning possible; to improve the quality of life for the participants among their peers; and to provide support and respite for the family and other caregivers to enable them to maintain veterans in the community. The care provided will include Physicians services, nursing, social work services, occupational or physical therapy, assistance with activities of daily living, activity therapy, case management, nutrition service, including meals, and transportation. The area of consideration is the Woodlands area near Houston, TX. Facility shall be a licensed ADHC facility. The contract will be awarded using the Best Value source selection process. Anticipated date for posting of the Request for Quote (RFQ) to FedBizOpps is February 16, 2015 and anticipated date for receipt of offers is March 2, 2015 4:00 PM CST. Offerors are advised they are responsible to check FedBizOpps website for any amendments which may be issued. All contractual and technical questions must be submitted in writing and emailed to Linda.Stevenson@va.gov. No telephone questions will be accepted. In accordance with FAR 52.204-7, prospective offerors shall be registered in the System for Award Management (SAM) database and IAW FAR 2.101 and VAAR 802.101; prospective offerors must be certified in the Vendor Information Portal (VIP) database prior to award. Offerors and contractors may obtain SAM information on registration and annual confirmation requirements via the Internet at http://www.sam.gov.
