- THIS IS A REQUEST FOR INFORMATION (RFI) ONLY TELEVISION COMMERCIAL TUSCALOOSA VA MEDICAL CENTER This is a request for information used to identify potential sources that are interested in and capable of performing the work described in the draft Government Statement of Work (see below). In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This RFI is issued solely for information and planning purposes and does not constitute a solicitation or obligation on the part of the government. Neither unsolicited proposals nor any other kind of offers will be considered in response to this RFI. No reimbursement will be made for any costs associated with providing information in response to this announcement and/or any follow-up information requests. STATEMENT OF WORK BELOW STATEMENT OF WORK Radio Commercial Tuscaloosa VA Medical Center I.BACKGROUND: The Tuscaloosa VA Medical Center will implement a multi-level outreach campaign utilizing paid advertisement to promote the Medical Center. TVAMC seeks to reach the largest amount of unique users possible to boost the enrollment in services at the Medical Center. Radio advertisement is an effective means of reaching a large number of people at one time. II.PURPOSE OF PROJECT: The purpose of the radio commercial ad purchase is to increase the community's awareness of the Tuscaloosa VA Medical Center as well as to increase the new users at the facility. The advertisements will serve as visual reminders for veterans to contact the VA to inquire about eligibility and enrollment. III.SCOPE OF WORK: a.Contractor shall provide adequate airtime of the pre-produced 30 second audio commercial, consisting of voices of TVAMC staff voicing a fictional storyline, on multiple local radio stations in the Tuscaloosa listening market. b.The advertisements should have a potential market reach of as many veterans in the West Alabama area with a concentration of those in Tuscaloosa County. c.The Tuscaloosa VA Medical Center requires that vendor have a listenership of at least 300,000 to effectively reach a wide population. d.Schedule of Deliverables: i.Period of Performance: March 1, 2015- June 30, 2015. ii.TVAMC will produce a 30 second commercial spot, consisting of TVAMC staff voicing a fictional storyline, to be aired on local radio station(s). iii.The ad will be engaging, legible, and reflect a standard of excellence. iv.Contractor will air the commercial at various times throughout the day on multiple stations to reach as many listeners as possible during the course of the contract. v.Statistical reports (including but not limited to demographic data and market reach date) shall be provided to the Tuscaloosa VAMC Point of Contact monthly via email. The statistical reports should identify the potential market reach of advertising during the term of the contract to enable the Tuscaloosa VAMC the ability to measure the effectiveness of the radio commercial advertising contract. vi.Proper destruction of digital files shall be completed by the contractor upon completion of the contract per Section VI of this document. All files, images, and graphics provided by TVAMC are the exclusive property of VA and are protected under the United States and International Copyright laws. The images may not be reproduced, copied, transmitted or manipulated outside of the intended purpose of this contract. IV.CONTRACTOR QUALIFICATIONS: a. Contractor must demonstrate the ability to provide all the requirements to meet or exceed the requirements as stated in the above Scope of Work. b. Contractor shall be registered in the System for Award Management (SAM) online system and have an active registration. V.GOVERNMENT FURNISHED PROPERTY: a. All digital audio files will be furnished to vendor by the Tuscaloosa VA Medical Center. It is understood that all files are property of and copyrighted by the Tuscaloosa VA Medical Center. b. Proper destruction of digital files shall be completed by the contractor upon completion of the contract per section VI of this document. All files, images and graphics provided by TVAMC are the exclusive property of VA and are protected under the United States Copyright laws. The images may not be reproduced, copied, transmitted or manipulated outside of the intended purpose of this contract. VI.RECORDS MANAGMENT: a. Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity's option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). b. No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. c. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. d. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. e. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. f. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. g. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. VII.CONTRACTOR PERSONELL SECURITY REQUIREMENT: a. The vendor will not have a need to be on site at the Tuscaloosa VAMC. They will not be required to access sensitive patient data or have access to Tuscaloosa VAMC data systems.
