Description

This is a Special Notice Synopsis. The Alexandria VA Health Care System (AVAHCS) Contracting Office intends to award a sole source contract, Basic Ordering Agreement (BOA), for Adult Day Care Services in the Stafford, TX area; zip code 77477 in support of the Michael E. DeBakey Veterans Affairs Medical Center in Houston, TX. The AVAHCS Contracting Office intends to award the sole source contract to It's Heaven Adult Day Health Care in Stafford, TX utilizing a BOA with a base year and four (4) option years of 12 months each. This BOA will be awarded on or about 17 February 2015 with an effective date of 20 March 2015. Period of Performances will be: BY: 20 March 2015 - 19 March 2016 OPT 1: 20 March 2016 - 19 March 2017 OPT 2: 20 March 2017 - 19 March 2018 OPT 3: 20 March 2018 - 19 March 2019 OPT 4: 20 March 2019 - 19 March 2020 This Sole Source is based on using FAR 6.302-5 - Authorized or Required by Statute. References include VHA Handbook 1141.03 - Adult Day Health Care; VHA Hand Book 1140.06 - Purchased Home Health Care Procedures; VHA Handbook 1140.01 - Community Residential Care Program; VHA Handbook 1143.02- Community Nursing Home Oversight Procedures; and the Veterans Access, Choice and Accountability Act of 2014. A Sole Source Justification and Approval is completed and approved. The negotiated rates are established by Medicaid and/or Medicare and cannot exceed established percentages above these rates as determined by the Department of Veteran Affairs and Medicare. Request of Proposal VA256-15-Q-0234 is the solicitation referenced for this action.
