Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation in its entirety; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) under solicitation number VA262-15-Q-0083. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-78 effective December 10, 2014, respectively. The solicitation is in accordance with FAR Part 12, 13, and 13.5, Acquisition of Commercial Items, Simplified Acquisition Procedures, Test Program for Certain Commercial Items, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. This is not a small business set-aside; the North American Industrial Classification System (NAICS) code for this requirement is 334516, and size standard 500 employees. The Department of Veterans Affairs Veterans Integrated Services Network (VISN) 22, headquartered in Long Beach, California has a requirement for a fully automated Immunohistochemistry (IHC) and in-situ hybridization (ISH) testing system. The Contractor shall provide the product for the entire VISN 22 at the following Veterans Affairs Healthcare Systems in Southern California: Long Beach (5901 E. 7th St. Long Beach, CA 90822), Greater Los Angeles (11301 Wilshire Blvd. Los Angeles, CA 90073), Loma Linda (11201 Benton St. Loma Linda, CA 92357), San Diego (3350 La Jolla Village Dr. San Diego, CA 92161), and Las Vegas (6900 N. Pecos Rd. North Las Vegas, NV 89086). Delivery is FOB destination. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of the blanket purchase agreement (BPA). The Government intends to award a single award BPA. The order shall begin on the date of award. **SEE ATTACHED SOLICITATION WITH ATTACHMENTS FOR SCHEDULE, PERIOD OF PERFORMANCE, DETAILS OF THE REQUIREMENT, CLAUSES, PROVISIONS, TERMS AND CONDITIONS** ADDITIONAL DOCUMENTATION REQUIREMENTS: Follow ALL directions and requirements within the solicitation. Incomplete submittals may be considered nonresponsive for failure to provide a complete submittal. In order to comply with the debt collection improvement act of 1966, all contractors must be registered and have an active registration in the System for Award Management (SAM) to be considered for an award of a federal contract. This system combines data that was formerly contained in the Central Contractors Registration (CCR) and Online Representation and Certifications (ORCA).
