FAR 8 - JUSTIFICATION FOR LIMITING SOURCES <$150K Requirement or RFQ Number: 531-15-2-950-0147 1.Description of Action: Nature: 1 New Requirement 0 Follow-on Order 0 Mod to Existing Order Number Order against: 1 FSS Contract Number: V797P-6019B Pricing: 1 Firm-Fixed Price 0 Time & Materials0 Cost Type Funds: 1 OMA 0 Other Funds: Name of Proposed Contractor:GE Healthcare Street Address:3000 N Grandview Blvd W-443 City, State, Zip:Waukesha, Wi 53188 Phone:919-244-8792 Name of Proposed Contractor: Street Address: City, State, Zip: Phone: 2.Description of Supplies or Services: The estimated value of the proposed action is NTE $25,000.00 The BVAMC's GE Definium 8000 X-Ray System Failed during during a patient study. This unit is integral to the BVAMC Radiology Department and this failure has led to a work stoppage. An order is required to troubleshoot and repair the unit to the Manufacturer's Operating Specifications as quickly as possible. 3.Authority: ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception: 1 An urgent and compelling need exists, and following the procedures would result in unacceptable delays(FAR 8.405-6(a)(1)(i)(A); 0 Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; (FAR 8.405-6(a)(1)(i)(B)) or 0 In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures. (FAR 8.405-6(a)(1)(i)(C) 4.Rational/explanation to support cited authority: The GE Definium 8000 X-Ray System failed during a patient study at the Boise VA Medical Center. The unit is an integral part of Boise VAMC's x-ray capabilities and immediate repair services are vital for continued healthcare delivery. In accordance with VHA Procurement Manual Chapter VI, 3.1.1, repairs to essential equipment that may preclude impairment of healthcare delivery capabilities or mission performance are deemed urgent and compelling. 5.BRAND NAME ONLY: This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-6(b)(3). 6.Note that this determination satisfies the requirements of FAR 8.405-6.
