Description
Statement of Work - Period of Performance may need to be adjusted. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NIHLM2015399 and is issued as a Request for Quotation (RFQ). The solicitation /contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification (NAICS) Code is 811219 with a size standard of 19.0 Mil This acquisition is being conducted using Simplified Acquisition Procedures and the Special Test Program in accordance with FAR Part 13. The National Institutes of Health (NIH), National Institute of Digestive, Diabetes & Kidney Diseases (NIDDK), and Division of Intermural Research Program (IRP) has a requirement to procure a preventative and maintenance service agreement for laboratory equipment used for research. The National Institutes of Health (NIH), National Library of Medicine (NLM), NIDDK Acquisitions Branch, Intramural Research Program (IRP) conducts biomedical research and training related to diabetes mellitus; endocrine, bone and metabolic diseases; digestive diseases, including liver diseases and nutritional disorders and kidney, urologic and hematologic diseases. During the course of research all of these departments require advanced fluorescence microscopy, including confocal and TIRF as the basis of a quantitative assay or to qualitatively complement other assays. The Carl Zeiss LSM 710 Scanhead, Scanning Module LSM 700 and Axio Observer Z1 Stand Mot were purchased to satisfy this need and need to be maintained for continued use. Equipment to be serviced: Carl Zeiss: 1) LSM 710 Scanhead S/N2504000141 M206027; 2) Scanning Module LSM 700 S/N:2601000384 M206032 3) Axio Observer Z1 Stand Mot S/N 3834004209 4) Scanning Stage 130x85 (cat 3834004209) 5) TIRF Slider 405-640nm with fibre mot (cat 3834004209) and High End Workstation Zeiss 45 A(0) (cat 3838894) Services to be performed: • Scheduled and Emergency service performed between the hours of 8:00am and 5:00pm Monday through Friday, excluding holidays. • All travel charges within the continental U.S.A. • One (1) * preventative maintenance inspection per Service Agreement year • All replacement parts, labor and technician travel • Applications Support/Training • Software updates when available • Factory trained Field Service • Proposed period of performance shall be 12 months from date of award. Place of Performance: National Institutes of Health, Building 8, 8 Center Drive, Bethesda, Maryland 20892. The offeror must include a completed copy of the following provisions: 1) FAR Clause 52.212-1 Instructions to Offerors - Commercial items; 2) FAR Clause 52.212-2, Evaluation - Commercial Items. As stated in FAR Clause 52.212-2 (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be advantageous to the Government, price and other factors considered. The following factors will be used equally to evaluate offers: Technical Evaluation, Price, and Past Performance. Note: Past Performance Information: Vendors must submit a listing of the most recent contracts/awards (minimum of 3) which demonstrate similar work in nature to this Solicitation. Contracts/awards may include those entered with the Federal Government, state and local governments and commercial concerns. Include the following information for each contract or subcontract: 1. Name of Contracting Organization 2. Contract Number (for subcontracts provide the prime contract number and the subcontract number) 3. Contract Type 4. Total Contract Value 5. Description of Requirement 6. Contracting Officer's Name and Telephone Number 7. Program Manager's Name and Telephone Number 3) FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items; 4) FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; 5) FAR Clause 52-212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions. The Dun and Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN) and the certification of business size shall be included. PLEASE NOTE: In order to receive an award, contractor must be registered and have valid certification in the System For Award Management (SAM) http://www.sam.gov Interested vendors capable of providing the Government with the items specified in this synopsis should submit their quotation to the below address. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis February 26 at 12:00 pm. EST. Offerors shall provide an original and two copies of your quotation. The quotation must reference Solicitation number NIHLM2015399. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the National Institutes of Health, National Library of Medicine, 6707 Democracy Blvd., Suite 700W, Bethesda, Maryland 20892, Attention: Linda Smith. Faxed copies will not be accepted. Submissions via email will be accepted.
