- Example of Receipt of Services Form Terms and Conditions Statement of Work Solicitation The Department of Transportation (DOT), Maritime Administration (MARAD) is the sponsor for this effort. MARAD seeks a qualified vendor to provide janitorial services for the Alameda Warehouse IAW the attached statement of work. (i) This acquisition uses the simplified procedures for acquisition of commercial items per FAR 13.5. This is a combined synopsis/solicitation for commercial items prepared In accordance with (IAW) the format at FAR subpart 12.6, Streamlined Procedures, under Acquisition of Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only written solicitation; quotations are being requested; and a written (formal) solicitation will not be issued. This solicitation and any amendments that include answers to questions will be provided through the FEDERAL BUSINESS OPPORTUNITIES (FBO) website https://www.fbo.gov. (ii) The solicitation number for this requirement is DTMA94Q1500012 and it is being issued as a Request for Quotation (RFQ). (iii) This solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-78 effective 26 Dec 2014. (iv) This procurement is a Total Small Business Set Aside for all responsible sources. Applicable North American Industry Classification System (NAICS) code is: 561720 Janitorial Services. MARAD intends to award a single fixed price Purchase Order (PO). (v) Instruction for Submittal of Quotation: a. Submit the items in the instruction below per FAR 52.212-1 Instructions to Offerors - Commercial Items. b. Submit your reps and certs as shown in the instruction below per FAR 52.212-3 Offeror Representations and Certifications - Commercial Items (MAY 2014). If you do not use the on-line certification you may do your reps and certs on the addendum to the RFQ that provides the full text clause. If you use the addendum then scan and submit your reps and certs with your quotation. (vi) Description of Requirements The requirement is for a vendor to provide janitorial services for the Alameda Warehouse IAW the attached statement of work. Attachment 1 Statement of Work Attachment 2 Terms and Conditions Attachment 3 Example of Receipt of Services Form (vii) Site Visit Although a site visit in not required, vendors are strongly encouraged to schedule a site visit in order to submit an informed proposal. Site visits can be schedule by contacting Mr. Mike Streblow @ 510-337-5084. Please allow at least 24 hours advance notice in order to schedule the visit. (viii) Questions regarding the solicitation and quotations shall be e-mailed to the Contracting Officer, Mr. Jon T. Meagher at jon.meagher@dot.gov. The cutoff date for questions is Wednesday February 18, 2015 at 1700 PST. No phone calls will be accepted. (ix) Quotations are due no later than Friday February 20, 2015 at 1400 PST. Submit quotes in FedBizOpps (FBO). Offerors may submit a courtesy copy of the quotation electronically to jon.meagher@dot.gov.
