Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number VA259-15-Q-0220 is issued as a request for quotation (RFQ). (2) This solicitation is for open market items under NAICS code 339113. (3) Contract Line Item number: 2 (4) Description of requirements for the items to be acquired: Line Item Product#DescriptionQty 100219500100MESHGRAFT II TISSUE EXPANSION UNIT3 EA 200219500600MESHGRAFT II AUTOCLAVE CASE3 EA Descriptions and Specifications This device works in conjunction with a skin removal device, Zimmer Mesh Graft dermatome, the Denver VA already owns. The Zimmer Mesh Graft II is compatible with our current dermatome system. (5) Delivery: 15 Days ARO, FOB Destination, Deliver To: VA Eastern Colorado Health Care System 1055 Clermont Street Attn: Warehouse (Angela Wheeler x 2949) Denver, CO 80246-6558 (6) Clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Product shall meet or exceed the specifications listed in the line item. Any substitution must provide detailed product descriptions and justification as to how the product deemed met or exceeded. No partial quote or shipment will be accepted. (8) Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (9) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (10) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition with consideration to 52.219-6, Notice of Total Small Business Set-Aside and 52.225-1, Buy American Act-Supplies.
