Description

The Department of Veterans Affairs, Tennessee Valley Healthcare System (TVHS) VA Medical Center (VAMC) has a requirement for a Capitated (Per Member/Per Month) Community Based Outpatient Clinic (CBOC) located within the city limits of Hopkinsville, KY in Christian County, KY. CBOC services include the provision of Primary Health (PH), Comprehensive Women's Health care, and Mental Health (MH) services in a private hospital, office or clinic environment to Veterans, primarily residing in the following Kentucky counties: Christian, Todd and Trigg. The proposed action will be a Firm Fixed Price, Indefinite Delivery, Indefinite Quantity (ID/IQ) contract. The North American Industrial Classification System (NAICS) Code applicable to this acquisition is 621498. The Small Business Size Standard for this acquisition is $20.5M. PERIOD OF PERFORMANCE: The period of performance for this procurement action is a base period of one (1) year, plus four (4) additional one-year option periods. A Request for Proposal (RFP) for this service is anticipated to be released on or about March 17, 2015. The solicitation will identify a due date for the submission of questions (approximately two (2) weeks after solicitation posting). Questions regarding this solicitation must be made in writing via email to the Contracting Officer, Kimberly Carson, at kimberly.carson@va.gov by the date stated in the solicitation. All potential offerors are encouraged to read over the entire solicitation package including attachments before submitting questions.
