Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This is a. (ii) The solicitation number is VA256-15-Q-0285 and is issued as a Request For Quotation (RFQ) (iii) Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as Service Disabled Veteran Owned Business (SDVOSB) Set Aside. The North American Industry Classification System (NAICS) code is 334516, and the size standard is 500 employees. (v) This requirement consists of one (1) line item: CLIN 0001: Cytation 5 Multi-Mode Reader or EQUAL/1/EA NOTE: Offerors submitting quotations on Equal items shall provide the Manufacturer's Name, Brand, Part Number, technical specifications, and images for the item. Weblinks will suffice for images and technical specifications. (vi) To be technically acceptable the products quoted shall be Brand Name or Equal to Cytation 5 Multi-Mode Reader and each additional item shall be comparable to what is offered with the Cytation 5 Multi-Mode Reader. (vii) Delivery FOB Destination to Anthony May, Cooper Division, 1101 Veterans Drive Lexington, KY 40502. Delivery is ARO +15 days (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; 52.216-1 (Firm Fix Price contract will be awarded), VAAR 852-215-70, 852.233-70; 852.233-71 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov.
