This RFQ does not commit Education & Training Resources (ETR) or Hartford Job Corps Academy to pay any costs incurred in the preparation of a quotation, nor does it commit ETR/HJCA to an award for products, supplies, or services. ETR/HJCA will be the sole judge as to which quotation offers the best overall value and will serve the best interest of ETR/HJCA. ETR/HJCA reserves the right to reject any and all Quotes without recourse from the vendor. Vendor must include proposed prices for base hourly rates and all overtime, weekend or holiday rates as applicable. Valid Business Licenses to operate in the state of Connecticut Statement of Background check or checks performed, and response statement to the Scope of Work provided. Address all Temporary employees services provided. (Administrative Assistant, Cafeteria, Wellness Departments) A certificate of general liability and professional liability insurance is required. SELECTION OF Temporary Employee Services: In addition to the Fee-For-Service pricing, selection of the vendor will be based on the following criteria: Experience in providing Temporary Employee Services for HJCA working with adolescents and young adults Provision of temporary employees for Administrative staff, Cafeteria staff and health services staff to HJCA youths who are socially and economically disadvantaged. Previous practice in residential settings such as schools, colleges, institutions, military, or correctional institution. Vendors willingness and ability to carry out required services duties and responsibilities Quality of temporary employee services made available under fixed price hourly quote. Approval of the temporary employee services vendor, BPA agreement and MOU by the ETR Corporate Office The most suitable temporary employee services vendor(s) may be interviewed prior to services award. BID PROVISIONS The following Provisions will apply. Failure to carefully read and follow all instructions and specifications will not be cause to negatively impact the center and are at the vendor's risk. All services and hourly prices must be typewritten without errors. All quotes/fee sheets must be having a company representative signature. Hourly Price Warranty: The Hourly Rates quoted herein are no higher than would be quoted to any other customer commercial and/or agencies or the United States Government for similar services. The Temporary Employee Services Vendor will list on a separate sheet of paper any exceptions to the conditions of this RFQ. This Fee/Quote sheet will be labeled "Exceptions to Quote Conditions" and will be attached to the Quote Sheet. If no exceptions are stated, it will be understood that all general and specific conditions will be complied with without exception. ETR reserves the right to accept and/or reject any or all quotes or waive any formality connected with the quotes. Lowest hourly rate or overall quote does not constitute an award. ETR will be the sole judge of which vendor Fee/Quotes is selected for approval (see "Selection of Temporary Employee Services" above) and BPA/MOU award. Items will be quoted F.O.B. Destination. No additional charge for delivery, drayage, express, parcel post, packing, cartage, insurance, license fees, permits, cost of bonds, or far any other purpose will be paid by ETR unless specifically included in the Fee/Quote original pricing and accepted by ETR. In the event that a vendor BPA/MOU is awarded in response to the RFQ, the below Statement of Work, provisions and set forth Terms & Conditions will be incorporated in their entirety into the BPA/MOU and become an integral part of the BPA/MOU when and if the BPA/MOU is accepted by Temporary Employee Services Vendor, either by acknowledgement or by commencement of performance. CENTER TEMPORARY EMPLOYEE SERVICES HARTFORD JOB CORPS ACADEMY Temporary Administrative Assistant Employee Temporary Cafeteria Employee Temporary Health and Wellness Employee SCOPE OF WORK The temporary employee services provider, referred to henceforth as the vendor, agrees to provide the Hartford Job Corps Academy, henceforth referred to as the center, with the temporary employee services stated herein. Services will be provided as authorized by the Center Director or his/her assigned buyers. Payment for services rendered will be according to the agreed upon BPA/MOU Fee-For-Service schedule. The base period of performance is Payment for services rendered will be according to the agreed upon Fee-For-Service schedule. The base period of performance is April 1, 2015 through March 31, 2016 with two (2) six (6) month options. The center and temporary employee services vendor hereby agree that the services to be provided will include, but will be limited to those services specified in attached scope of work and the Job Corps Policy Federal Regulations (Title 20 Code of Federal Regulations, Part 638.510) in the Job Corps Policy & Requirements Handbook (PRH) Chapter 6, and applicable Technical Assistance Guides (TAGs), www.jobcorpshealth.com. Services: Vendor shall provide temporary employee services to the Hartford Job Corps Academy, as requested by the Administrative Manager. Cafeteria Manager or Health & Wellness Manager (HWM) or designee. Vendor shall provide quality temporary employee services to fill vacancies and replace a HJCA employee. Temporary Employee provided by the vendor shall meet all required HJCA background checks and supply documentation needs per HJCA department managers. (Licenses, Certifications, Qualifications.) Temporary employee services vendor must also supply the Hartford Job Corps Academy with certified timesheets for all temporary employee hours' work by the temporary employee. Temporary Employee Vendor must abide by all HIPPA policies including posting a Job Corps HIPPAA Notice in the Temporary Employee Vendors Office. Temporary employees Food Service Provide the below listed temporary kitchen staff in support of HJCA. Request may be with less than 12 hour notice. Cook -Prepare meals following published menus for 80-150 students. Assist in serving meals, adhere to all State guidelines concerning food safety and proper food handling procedures, and cleans assigned area. Must have current ServeSafe certificate. Experience in a cafeteria/food service setting preferred. Food Service Assistant -Cleans and maintains dining room and kitchen facilities. Washes dishes, glassware, flatware, etc. Observes the food safety requirements. Experience in a cafeteria/food service setting preferred. Requested support will be for 4-8 hours daily during the following times: M-F 6:00 AM - 7:00 PM S-S 9:30 AM - 6:30 PM Worker must have current criminal background check and be drug free. Administrative Assistant Provide temporary office staff to provide administrative support to HJCA departments. Request may be with less than 12 hour notice. Duties include filing, data input, and answering phones, provide customer service, etc. Requires basic computer and phone skills. Knowledgeable in MS Office Suite. Duty will be during 8 hours daily during weekdays, 8:00 AM - 5:00 PM, with 1 hour unpaid lunch. Requested support will be for 8 hours daily during the following times: M-F 8:00 AM - 5:00 PM Worker must have current criminal background check and be drug free. Temporary Health and Wellness Employee LPN- Responsibilities: Conduct cursory medical examinations, preform triage and first aid treatment as needed, collect specimen, administer medication, provide training to staff and students, schedule outside medical appointment, and maintain treatment plans Minimum 3 years experience. Experience working with youth preferred. State of Connecticut Certification. Valid Drivers License Requested support will be for 8 hours daily during the following times: M-F 8:00 AM - 5:00 PM
