For questions and/or clarifications contact Paulette Kennedy 215-823-5800 x4669 STATEMENT OF WORK SIP TRUNKING TO SUPPORT DICTATION SYSTEM A.GENERAL INFORMATION (1)Scope of Work: The scope of work includes all acquisition, labor, materials, programming, licenses, supervision and quality assurance necessary to install 80 SIP trunks into the existing Mitel 3300 ICP telephone system which will integrate with our new Fusion dictation voice system. (2)Background: Our new dictation system is configured for Voice-Over-IP (VOIP) integration. This requires our Mitel phone system to connect via SIP trunking so callers can access this system. B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS (1)The contractor shall supply the following:. Licenses for SIP trunking - quantity 80 oLabor and travel to install, configure, program the SIP licenses and trunking in the Mitel 3300 ICP to integrate with the Fusion voice dictation system. oThe requirement will be for an on-site installation. Remote access will not be provided. (2)INSTALLATION- The Contractor shall provide a system Installation Plan (IP) to the Contracting Officer Representative (COR) for approval prior to the start of work. The COR shall approve the IP and return the contractor. The Installation Plan shall include estimated timeline of start and completion and estimate unexpected delays/lead time. a.Perform staging, programming, and configuration of all system infrastructures. b.Perform all necessary test/audits on all system infrastructures. (4)TEST AND ACCEPTANCE: Contractor shall test all equipment after installation. The government shall accept equipment once installation and successful testing has been completed and approved. (5)WARRANTY/SERVICE CONTRACT: All equipment and materials shall come with a standard one year warranty. Warranty shall begin after installation of equipment and completion of tests. D. CONTRACTOR RESPONSIBILITIES: The contractor shall be responsible for the following: (1)Contractor shall provide certified Mitel technicians to perform all installations. (2)Contractor shall be responsible for all system/software testing after installation and prior to government acceptance. (3)Contractor is responsible for ensuring the proper disposal of all debris generated from installation activities. (4)Contractor is responsible for securing all materials, equipment and tools while on government property or in government facility. Government is not liable for any lost or stolen items that are not properly secured. E. GOVERNMENT RESPONSIBILITIES: The Government shall provide the following: (1)Provide site access and escorts through work areas as needed. (2)Provide primary electrical power. (3)Provide adequate space to perform the work. F. OTHER CONSIDERATIONS: (1)CONTRACTING OFFICER REPRESENTATIVE (COR): The Contracting Officer Representative shall be the technical point of contact for all work related requirements. The COR does not have authorization to change, alter, or remove any requirements stated in the performance work statement. The Contracting Officer is the only government authorized person to change the performance work statement or any resultant contract. (2)PLACE OF PERFORMANCE: The place of performance is the VA Medical Center, 1700 S. Lincoln Ave., Lebanon, PA, 17042 (3)WORK HOURS: Operational Hours are 0730-1600, Monday-Friday
