Description

The Department Of Veterans Affairs, National CMOP Office of NCO 15 has a requirement to procure pharmaceuticals for delivery to the Midsouth CMOP in Murfreesboro, TN. The request is for (29 Line items) Miscellaneous Pharmaceuticals to replenish their stock. The Product Code is 6505 (Drugs and Biologicals) The NAICS code for this solicitation is 325412. Submit the "pricing schedule (764)" of the RFQ via email to Jennifer.arellano@va.gov No solicitation packages will be accepted by snail mail or by facsimile. The phone number for solicitation questions is (913) 758-4761. Solicitation package VA770-15-Q-0016 shall be posted on Fedbizopps on or about February 13, 2015; and will close at 1700 CST on February 18, 2015. Vendors must be able to deliver item(s) NLT 5 days ARO; or by way of multiple deliveries with the first ½ of all line items NLT 5 days ARO and the remaining ½ NLT 20 days ARO. The columns for "Items in Current Inventory" and "Country of Origin" must be filled in to be considered acceptable. Vendors must adhere to "The Drug Supply Chain Security Act"; and submit the required information with shipment(s) effective January 1, 2015. Responses must be concise and be specifically directed to the requirement referenced above. It is the offertory's responsibility to monitor FedBizOpps for changes or amendments. Must specify which NDC number you are quoting; otherwise the NDC number which is on the same line as your quote will be used. Vendor shall list the time frame in which ALL quotes are good; if not specifically listed on the RFQ then quotes will be considered good for 15 days after the Solicitation closes. It is the vendor's responsibility to ensure the quotes are accurate when they submit them. Price changes will not be made after award if it is within the 15 days!!!
