- Solicitation Number: VA258-15-R-0215 CSS Notice Type: Combined Synopsis / Solicitation Synopsis: This combined synopsis/solicitation is a request for proposal (RFP). This RFP is being utilized to seek out interested vendors for Radiation Dosimetry Badge Service (also known as film badge service) for four hospitals located in Texas, New Mexico, and Arizona. NAICS Code: 541380 - Testing Laboratories Small Business Size Standard: $15.0 million / year Responses: Interested Vendors shall respond by completing the Standard Form 1449 section 17a, 30a, and 30b. Responses to the RFP shall be based on the material contained in this RFP. The basis of award shall be Lowest Price Technically Acceptable (LPTA). Interested Vendors are highly encouraged to submit proposals in accordance with the Addendum to 52.212-2 Evaluation factors | Instructions to Offerors. It includes information to guide the content of your response and requests basic information about your capabilities and experience in providing the services listed. Responses should be based on the material contained in this solicitation. Responses will not be returned. Telephone responses will not be accepted. QUESTIONS: Questions and responses shall be submitted by email or mailed to Garland D. Nichols, Contract Specialist at garland.nichols@va.gov with the email subject heading as "Radiation Dosimeter Badge Service". The proposal can also be mailed to SAO West VISN 18, Contracting Office, Garland D. Nichols, 7201 I-40 West, Suite 100, Amarillo, TX 79106. The response shall be submitted to the Contract Specialist no later than March 2, 2015, 10:00 am CST.
