Description

W-9 Vendor Questionnaire Scope of Work--Dental Services THIS IS A SUBCONTRACTING OPPORTUNITY WITH THE GADSDEN JOB CORPS CENTER/ADAMS AND ASSOCIATES AND NOT WITH THE U.S. DEPARTMENT OF LABOR The Center Dentist will provide students of the Gadsden Job Corps Center with the services stated below. Such services will be provided as authorized by the Center Director or his/her representative Services to be provided will be in the amount of eight (8) students per week for 4 weeks, including: dentist, dental assistant and hygienist services The Base period of performance will be from March 1, 2015 through March 31, 2015. This period is subject to change Services to be provided will include, but will not be limited to those oral health and wellness services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510; the Job Corps Policy and Requirements Handbook, Chapter 6, Section 6.10, R2; and applicable Technical Assistance Guides).
