- FAR 52.212-3 Offer Representation and Certifications in full text Past Performance Questionnaire Site Visit Instructions Entry Access List Wage Determination PWS Combined Synopsis/Solicitation Paint Blast Booth (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested. (ii) This solicitation/synopsis reference number is a Request for Quotations (RFQ) and the reference number is FA4686-15-T-0005. Written quotes are required (oral offers will NOT be accepted). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-78, effective 26 Dec 2014 (Includes FAC 2005-79 effective 15 Dec 2014). (iv) This acquisition is to be competed 100% Small business set-aside. The associated North American Industrial Classification Standard (NAICS) code is 811310, Commercial and Industrial Machinery and Equipment, with a size standard of $7.5M. The standard industry code is 7699. (v) The United States Air Force, 9th Contracting Squadron, Beale AFB, California, intends to award a service type contract In Accordance With (IAW) the attached Performance Work Statement (PWS), bid schedule, and all other solicitation attachments. See attached bid schedule for a full list of contract line item number(s) and items, quantities and units of measure to include all including option(s). (vi) See PWS for description of the requirement(s). (vii) The resultant award will provide services to Beale AFB, CA. See the bid schedule attachment for the Period of Performance. Please see the provision below: Instructions to Offerors - Commercial Items (Addenda to FAR 52.212-1, APR 2014) (b)(4) To determine technical capability, each offeror shall submit a capability statement not exceeding 10 pages. The capability statement provided shall address the capability of the contractor to perform the work required in accordance with the PWS. (b)(10) The offeror shall submit a listing of relevant past contracts (minimum of two) pertaining to its experience over the past three (3) years in performing the maintenance services as described in the PWS. The list shall contain the contract number, dollar value, description of the project, points of contact (name, address, telephone number and email address) and cite any challenges encountered and overcome. The Past Performance Questionnaire (PPQ), provided in this solicitation, shall be sent by the offeror to their customers to allow for sufficient time for completion and inclusion in the offeror's offer. The PPQ is a method of collecting this information however, past performance information may be obtained from any other sources available to the Government to include, but not limited to, SAM, FAPIIS, CPARS, PPIRS, or other databases, interviews with Program Managers, Contracting Officers and other Government representatives. The offeror shall submit a capability statement along with their proposal to prove that they are capable of providing the services required per the performance work statement (PWS). 3) Past Performance Questionnaire (PPQ) - Contractor shall provide recent and relevent evidence (example of evidence: past performance on similar machines per PWS, certifications, etc) of having provided the required maintenance on machines listed on the PWS (section 2, page 1). The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. As used in this clause- (1) "Over and above work" means work discovered during the course of performing overhaul, maintenance, and repair efforts that is- (i) Within the general scope of the contract; (ii) Not covered by the line item(s) for the basic work under the contract; and (iii) Necessary in order to satisfactorily complete the contract. (2) "Work request" means a document prepared by the Contractor which describes over and above work being proposed. (b) The Contractor and Administrative Contracting Officer shall mutually agree to procedures for Government administration and Contractor performance of over and above work requests. If the parties cannot agree upon the procedures, the Administrative Contracting Officer has the unilateral right to direct the over and above work procedures to be followed. These procedures shall, as a minimum, cover- (1) The format, content, and submission of work requests by the Contractor. Work requests shall contain data on the type of discrepancy disclosed, the specific location of the discrepancy, and the estimated labor hours and material required to correct the discrepancy. Data shall be sufficient to satisfy contract requirements and obtain the authorization of the Contracting Officer to perform the proposed work; (2) Government review, verification, and authorization of the work; and (3) Proposal pricing, submission, negotiation, and definitization. (c) Upon discovery of the need for over and above work, the Contractor shall prepare and furnish to the Government a work request in accordance with the agreed-to procedures. (d) The Government shall- (1) Promptly review the work request; (2) Verify that the proposed work is required and not covered under the basic contract line item(s); (3) Verify that the proposed corrective action is appropriate; and (4) Authorize over and above work as necessary. (e) The Contractor shall promptly submit to the Contracting Officer, a proposal for the over and above work. The Government and Contractor will then negotiate a settlement for the over and above work. Contract modifications will be executed to definitize all over and above work. (f) Failure to agree on the price of over and above work shall be a dispute within the meaning of the Disputes clause of this contract. DFARS 252.232-7006 Wide Area WorkFlow Payment Instructions (a) Definitions. As used in this clause- "Department of Defense Activity Address Code (DoDAAC)" is a six position code that uniquely identifies a unit, activity, or organization. "Document type" means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF). "Local processing office (LPO)" is the office responsible for payment certification when payment certification is done external to the entitlement system. (b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports. (c) WAWF access. To access WAWF, the Contractor shall- (1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and (2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site. (d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the "Web Based Training" link on the WAWF home page at https://wawf.eb.mil/ (e) WAWF methods of document submission. (End of clause) AFFARS 5352.201-9101 Ombudsman; (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. AFFARS 5352.223-9000 ELIMINATION OF USE OF CLASS I OZONE DEPLETING SUBSTANCES (ODSs) (NOV 2012) (a) Contractors shall not: (1) Provide any service or product with any specification, standard, drawing, or other document that requires the use of a Class I ODS in the test, operation, or maintenance of any system, subsystem, item, component, or process; or (2) Provide any specification, standard, drawing, or other document that establishes a test, operation, or maintenance requirement that can only be met by use of a Class I ODS as part of this contract/order. [Note: This prohibition does not apply to manufacturing.] (b) For the purposes of Air Force policy, the following products that are pure (i.e., they meet the relevant product specification identified in AFI 32-7086) are Class I ODSs: (1) Halons: 1011, 1202, 1211, 1301, and 2402; (2) Chlorofluorocarbons (CFCs): CFC-11, CFC-12, CFC-13, CFC-111, CFC-112, CFC-113, CFC-114, CFC-115, CFC-211, CFC-212, CFC-213, CFC-214, CFC-215, CFC-216, and CFC-217, and the blends R-500, R-501, R-502, and R-503; and (3) Carbon Tetrachloride, Methyl Chloroform, and Methyl Bromide. [NOTE: Material that uses one or more of these Class I ODSs as minor constituents do not meet the Air Force definition of a Class I ODS.] AFFARS 5352.223-9001, Health and Safety on Government Installations (NOV 2012) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract. AFFARS 5352.237-9000, Contractor Access to Air Force Installations (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. AFFARS 5352.237-9000 Contractor Access to Air Force Installations (deviation) Paragraphs (b) and (f) of AFFARS 5352.242-9000 shall be replaced in their entirety with the following: (b)(1) To obtain entry to Beale AFB, Contractors shall submit the following information (for each of the Contractor's personnel) to 9 CONS/LGCA a minimum of 72 hours prior to the requested entry date: a. Full name b. Driver's license number and State of issue c. Home address, city, state and zip code d. Home telephone number e. Description of vehicle to be driven on-base (year, make, model, license number) f. Date on which the individual will be entering Beale AFB (b)(2) Contractor entry requests three full business days (72 hours) for Security Forces to process. Business days do not include weekends and the following federal holidays New Year's Day Labor Day Martin Luther King's Birthday Columbus Day President's Day Veteran's Day Memorial Day Thanksgiving Day Independence Day Christmas Day (b)(3) In addition to a contractor pass, all individuals must have a valid picture identification (valid driver's license or other Government-issued identification) in their possession while they are on Beale AFB. Vehicles entering Beale AFB shall be marked with the Contractor's (or sub-contractor's) insignia or signage. Individuals driving vehicles on Beale AFB must have the following documents in their possession for the vehicle they are driving: a. Current vehicle registration b. Proof of valid insurance coverage c. Valid driver's license (b)(4) When base entry for sub-contractors is necessary, all requirements and procedures identified in this provision shall apply and be followed. In addition to the information required in (b)(1), the sub-contractor's company name, address, and telephone number shall be provided for each of the sub-contractor's personnel. (b)(5) Deliveries of equipment and materials (e.g., concrete, heavy/light equipment, supplies, etc.) are included in the 72-hour advance notification requirement. In some instances, the normal scheduling practices do not allow for identification of the delivery driver in advance of the day of delivery. These instances will be coordinated with 9CONS/LGCA on a case-by-case basis. However, 72-hours advance notification of the delivery (company name, address, and telephone number as well as description of the delivery) is required. (b)(6) As real-world events change, these requirements and procedures are subject to change without notice. To the extent possible, the Government will inform the prime Contractor of any changes as they occur. Changes in these procedures due to Beale AFB acting in its capacity as a sovereign shall not be the basis for claims against the Government. (f)(1) Prior to submitting an invoice for final payment, the prime contractor shall obtain a clearance certification from the issuing office which states all base identification passes have been turned in, accounted for, or transferred to a follow-on contract. This certification shall be submitted to the Contracting Officer prior to submission of the final invoice for payment. (f)(2) If an employee of the Contractor or sub-contractor leaves employment, the Contractor shall ensure that the Government-issued identification and/or contractor pass possessed by that employee is returned to the Contracting Officer as soon as possible. (g) Failure to comply with these requirements may result in withholding of final payment. (h) In accordance with BAFBI 31-101 and BAFB Policy Letter #4: Individuals whose criminal background meets any of the following criteria shall not be allowed access to Beale AFB: a. The individual is required to register as a sexual offender under federal law or the applicable state law; b. The individual has been convicted of illegal possession or use of drugs/narcotics on more than one occasion within five years from the date access to Beale AFB is requested; c. The individual has been convicted of any offense involving drug trafficking within 10 years from date access to Beale AFB is requested; d. The individual has been convicted of a felony involving violence against a person, arson, robbery, or burglary within 5 years from the date access to Beale AFB is requested; or e. The individual has been convicted of any crime involving incident acts with a minor, or a felony that is sexual in nature. Individuals who meet any of the criteria stated above shall be allowed to submit an application for waiver of this policy to the Beale AFB Contracting Office.
