Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit offers. Solicitation VA-249-15-Q-0305 is issued as a Request for quote (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. This requirement is a Small Business Set Aside. The NAICS Code is 621511 and size standard is $30 million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. The government anticipates awarding a Firm Fixed Price award. The bases of award will be Lowest Price Quote. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements and will provide the best value to the government. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all services required to successfully complete laboratory reference tests listed in the attached statement of work. This service is required for the Nashville, Tennessee VA Medical Centers, at 1310 24th Ave South Nashville TN 37212 within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein. Submission of Quote: The Offeror shall submit their quote on company letterhead and shall include unit price, total, unit quantity, terms of any express warranty, unit price, overall total price, applicable shipping charges and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov. Complete copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. The offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The offeror shall provide past performance evidence. This assessment is based on the offeror's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. All Quotes regarding this solicitation must be submitted to the Contract Specialist in writing by e-mail to Scheronica.Cochran@va.gov no later than 8:00 AM CST, February 27, 2015. The subject line must specify VA249-15-Q-0305 - Reference Testing at the VAMC Nashville. The solicitation package will be posted on FedBizOpps, which can be accessed at www.fedbizopps.gov. The Government will not provide paper copies of the solicitation. Statement of Work Specialized Cancer Testing 1.Requirements TVHS needs a vendor to perform the following special cancer tests: Oncotype DX Breast Cancer Assay, the Oncotype DX Colon Cancer Assay and the Oncotype DX Prostate Cancer Assay. The required testing will analyze the activity of a group of genes that can affect how a cancer is likely to behave and respond to treatment. Testing results need to show how complex genes are expressed in cells (what their level of activity is), and the role they play in specific cancers including breast, colon, and prostate. 2.Scope. Base Plus Four Year Options. Blanket Purchase Agreement (BPA) of cancer testing, with the required tests listed in the chart under "Specific Tasks." TVHS will only accept tests of equivalent or higher performance to the Oncotype DX test. Equivalent tests must show clinical evidence and at least three years on the market in labs (outside of research purposes). The VA TVHS requires testing that has been reliable for years and will not risk experimenting with newer specialized cancer tests unless evidence guarantees greater benefits to Veterans. The requested tests have been proven to predict the likelihood of cancer recurrence, the likelihood of patient survival and the likelihood of chemotherapy benefit. These unique diagnostic tests can help patients and their doctors make informed, individualized treatment decisions. Results will also help clinicians determine if the patient needs to undergo chemotherapy treatment or not, saving the VA thousands of dollars in treatment costs. 3.Specific Tasks (Evaluation Factors). The Reference Laboratory shall provide in accordance with all applicable federal, state, and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, patient testing to aid in the treatment of cancer. Estimated quantities of tests will depend upon patient specifics. Vendor must offer all tests listed in the test menu below. Total price must include, labor, travel and delivery. TVHS will only accept tests of equivalent or higher performance to the Oncotype DX test. Equivalent tests must show clinical evidence and at least three years on the market in labs (outside of research purposes). TEST MENU: Following tests must be available from vendor Oncotype DX Breast Cancer Assay Oncotype DX Colon Cancer Assay Oncotype DX Prostate Cancer Assay 4.Performance Monitoring Performance monitoring will be done by a third party, the organization that the reference lab has accreditation through. The reference lab will maintain on file their current accreditation and make it available for review upon request. 5.Other Pertinent Information or Special Considerations. a.Identification of Possible Follow-on Work. All parties involved with the review and establishment of this request should not have any conflicts of interest with the chosen vendor. c.Identification of Non-Disclosure Requirements.
