Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-15-Q-0197 is issued as a request for quote (RFQ). The North American Industry Classification System (NAICS) Code 334510 "Electromedical & Electrotherapeutic apparatus Manufacturing". Business Size Standard of 500 employees is applicable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-78. No telephone requests will be accepted. Quote is requested for: ASL 5000 Breathing Simulator. SPECIFIC CONTRACT REQUIREMENTS: Contractor shall provide the supplies/equipment, per the attached solicitation VA251-15-Q-0197 at the Department of Veteran Affairs VA Medical Center, 1481 W. Tenth Street, Indianapolis, IN 46202. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) to VA Financial Services Center, P.O. Box 149971, Austin, Texas 78714-8971 through the OB10 electronic invoicing system. EVALUATION CRITERIA Award of this order will be made on a competitive best value basis, using "tradeoff" among cost/price and non-cost/price factors. The Government may elect to award to other than the lowest priced offeror, or other than the offeror with the highest rated non-cost/price proposal. In either case, a tradeoff will be conducted. The government reserves such right of flexibility in conducting the evaluation as necessary to assure an award with the contractor providing the best value to the government. The cost/non-cost price evaluation factors numbered in descending order of importance are as follows: 1.Small Business 2.Technical 3.Price 4.Pass Performance Full text of the following clauses and/or provisions can be viewed through Internet access at http://farsite.hill.af.mil. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items or confirm this has already been completed in their SAM registration. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34. Solicitation documents are attached to this notice. Offerors shall submit their quotation by e-mail to Joyce.Miller@va.gov, no telephone inquiries will be taken. Offers are due no later than 3:00 pm. EST, March 16, 2015. The successful offeror must be registered with SAM to be awarded a contract. You may register on-line at https://www.sam.gov.
