- This is a Combined Synopsis/Solicitation Notice as defined in FAR 15.201(e). The Department of Veterans Affairs, Network Contracting Office 16, MICHAEL E. DEBAKEY VA Medical Center in HOUSTON, TEXAS 77030 has a requirement for LumenisPulse Laser Stk# GA-0006800. This is a Combined Synopsis/Solicitation Notice for information and planning purposes and shall not be construed as an obligation on the part of the Department of Veterans Affairs. The purpose of this Combined Synopsis/Solicitation Notice is to identify qualified businesses, including Service-Disabled Veteran-owned Small Business (SDVOSB), Veteran-owned Small Business (VOSB), Small Disadvantaged Business (SDB), Women-owned Small Business (WOSB), Historically Underutilized Business Zone (HUB Zone) Small Business, 8(a) Small Business, and small business concerns that are interested in and capable of providing the items listed herein. The NAICS Code for this procurement is 339112 and the corresponding size standard is 500 Employees. Your response to this sources sought will assist the Government in determining the appropriate acquisition method. The items must be compatible with Veterans Affairs standards. The following items are sought: Item No./Stock No. Description Quantity Unit 1.) LUMENISPULSE LASER HOLMIUM AT 2,100 NM AND/OR 0.2-6.0 JOULES FOR A TOTAL POWER OF UP TO 120 WATTS. 1 EA 2.) TOTAL PROTECTION PLAN (WARRANTY) 4 YR Responses to this notice should include the following: company name, address, point of contact, phone and fax numbers, email address, DUNS number, capability statement, business size and any other pertinent information that demonstrates the firm's ability to meet the Government' s requirement. All responses shall be submitted in writing no later than 10 MARCH 2015 at 2:00 pm CST to the attention of THOMAS JOHNSON. THOMAS.JOHNSON9@VA.GOV Telephone responses will not be accepted. This notice shall not be construed as a commitment by the Government to issue a solicitation or make an award, nor does the notice restrict the Government to a particular acquisition approach. A response to request for information is not a request to be added to a prospective bidder's list or to receive a copy of the solicitation. Interested offerors must respond to the solicitation, if issued, in addition to responding to this request for information.
