- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W912L7-15-Q-0401 is hereby issued as a Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-79. This requirement is being advertised as unrestricted. The Tennessee Army National Guard has a requirement for the following vaccines: 1. HAVRIX SYRINGE 1M 1440IU/ML25 EACH@$__________/EA TOTAL: ____________ 2. TYPHIM VI TTYPHOID PREF SOI-H250 EACH@$_________/EA TOTAL: ____________ 5. SHIP ON ICE CHARGE 1 EACH@$_________/EA TOTAL: ____________ 6. EXCISE TAX IF APPLICABLE 1 EACH@$_________/EA TOTAL: ____________ The North American Industry Classification System (NAICS) code is 325412 with a small business size standard of 750 employees. All responsible sources may submit a proposal, which shall be considered by this Agency. The following clauses may be applicable to this requisition if proposals are open market: FAR 52.211-6 Brand Name or equal, FAR 52.212-1 Instructions to Offerors Commercial Items, FAR 52.212-2 Evaluation Commercial Items, FAR 52.212-3, Offeror Representations and Certifications - Commercial Items; FAR 52.212-4, Contract Terms and Conditions-commercial items, FAR 52.212-5(DEV) Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (JUN 2006) (Deviation) and DFARS 252.212-7001 Contract Terms, Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items (JUL 2006)(Deviation), DFARS 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. If proposing on an equal, the equal it must meet the Salient Characteristics of the above items. Award will be determined in the best interest to the Government based on best value, by product compatibility with salient characteristics, lowest price, technically acceptable.
