- BACKGROUND. Recruits in Basic Combat Training (BCT) have the highest incidence of musculoskeletal injuries of any subgroup in the US Army. This not only affects Soldier preparation, but also results in medical attrition which reduces military manpower. Therefore, the US Army has decided to establish a program providing forward musculoskeletal care to the trainees in the Initial Military Training (IMT) environment. Forward musculoskeletal care minimizes lost training time and ensures early identification of injuries. This increases the likelihood of timely graduation and raises the overall quality of trainees merging into the Army. Reducing injury-related attrition is a multi-factorial task that includes, but is not limited to, command emphasis, training, and enforcement of evidence-based injury prevention (IP) techniques. Well-trained, combat-ready Soldiers are best produced by allocating healthcare assets that optimize physical performance, prevent injury, and facilitate early identification and management of injuries when they do occur. Soldiers in BCT and One Station Unit Training Brigades (OSUT) perform repetitive physical tasks, such as, foot marching, drill and ceremony, rifle marksmanship, and repeated obstacle course circuits, resulting in a high number of overuse-type injuries. The types of injuries commonly seen in Army IMT are very similar to those commonly seen in athletics, including sprains, strains, tendonitis, and stress fractures. To provide forward musculoskeletal care and early intervention services, the Army MEDCOM will contract athletic trainer services for designated Training and Doctrine Command (TRADOC) Initial Entry Training (IET) sites to provide limited forward and clinic based musculoskeletal care to trainees. The Athletic Trainers (AT) providing health care services shall perform duties under the appropriate clinical supervision of a government Physical Therapist with physician oversight as required by AT licensing regulations. All health care services performed on federal installations are in compliance with Department of Defense (DoD) Manual Number 6025.13, Medical Quality Assurance (MQA) and Clinical Quality Management in the Military Health System (MHS); and Army Regulation 40-68, Medical Services, Clinical Quality Management.
