THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs, Augusta GA, is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work required under the proposed contract. LOCATION: The Charleston VA Medical Center, Charleston SC, and the Dublin VA Medical Center, Dublin GA, requires home oxygen and associated services to Veteran beneficiaries. The contractor shall provide home oxygen within a defined area of responsibility/jurisdiction to the patients' residence, or other location as indicated on the order for home oxygen. The area of responsibility/jurisdiction for each VA facility shall include the counties identified as applicable to the area of responsibility/jurisdiction. Contractor shall provide setup, installation, and management of home oxygen services to any patients that are treated at the Charleston and Dublin VA Medical Centers and all Community Based Outpatient Centers (CBOCs) supported therein (Clinic of Jurisdiction/Primary Service Area( COJ/PSA)) regardless of their physical location. The counties serviced for Charleston are as follows: BEAUFORT, BERKELEY, CHARLESTON, COLLETON, DORCHESTER, GEORGETOWN, WILLIAMSBURG, HAMPTON, HORRY, JASPER, & MARION. THE SERVICING COUNTIES IN GEORGIA ARE AS FOLLOWS: BRYAN, CHATOM, MCINTOSH, WAYNE, EFFINGHAM, LIBERTY, & LONG. The counties serviced for Dublin are as follows: APPLING, BACON, BAKER, BALDWIN, BEN HILL, BIBB, BLECKLEY, BRYN, BULLOCH, CALHOUN, CANDLER, COFFEE, CRAWFORD, CRISP, DODGE, DOOLY, DOUGBERRY, EMANUEL, EVANS, GLYNN, HOUSTON, IRWIN, JEFF DAVIS, JOHNSON, JONES LEE, LIBERRY, LONG, MCINTOSH, MACON, MONROE, MONTGOMERY, PEACH, PULASKI, PUTNAM, SUMTER, TATTNAIL, TAYLOR, TELFAIR, TERRELL, TIFT, TOOMBS, TREUTLEN, TURNER, TWIGGS, WASHINGTON, WAYNE, WHEELEY, WILCOX, WILKINSON, WORTH, LAURENS. A stock of ten (10) portable cylinders, regulators and 02 carriers shall be continuously maintained by the contractor at the VA Medical Centers located in Charleston SC and Dublin GA. These cylinders shall be used to accommodate home oxygen beneficiaries who are visiting the VA or being discharged and require a tank of oxygen to go home. This service shall be provided at no additional cost to the Government. The VA shall furnish storage space and maintain all security for this equipment. The Contractor shall be permitted access to the space and equipment when requested by telephone with 24 hours' notice to the VA Medical Center's prosthetics Service. Contractor shall maintain an updated inventory of all equipment on a quarterly basis. NAICS: 532291 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by March 26, 2015 to: Department of Veterans Affairs, VA Southeast Network - 501 Greene St, Hatcher Building, Suite 2, ATTN: Shirley Adams, Augusta, GA 30901-4401. Responses should be received no later than 2:00 PM EDT. Submissions must be made via email to: Shirley.Adams@va.gov or FAX: (478) 274-5588. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER March 26, 2015. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. (5) No Proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.)
