Description
Important Date: Please submit final quote via email to brooke.hansen@va.gov by 10:00am on Monday, March 9, 2015. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA251-15-Q-0206 and is issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 333993 with a small business size standard of 500 employees. This is a small business set-aside. Please submit all offers to Brooke.Hansen@va.gov before Monday, March 9, 2015 at 10:00am Eastern Time. FOB Destination shall be Battle Creek, MI 49037. The VHA NCO 11 - 515 - Battle Creek, MI requires the following items, Exact Match Only, to the following: AP-2-9300-2-115Auto-Print - 2.0" Standard2.0" Unit Dose Packaging System for Oral Solids. PF-1 Auto-Print Oral Solid Feeder - features a revolutionary new design, state-of-the-art technology and FDA approved materials and coatings. This rotary feeder adapts to MPI's Auto-Print streamlining the packaging process for oral solids by aiding in the efficiency in which tablets and capsules are packaged into unit dose. Bulk medication is loaded into the feeder which will automatically feed the medication one at a time until all medications have been packaged thus eliminating the need for manual hand feeding by the pharmacy technician and increasing sanitary conditions while packaging. When the medication supply is depleted, the Auto-Print Pill Feeder will automatically pause and notify the user with a momentary audible alert. VL-1Variable Package Length Option - adds the capability to change the package length as needed. Pre-loaded software templates are available for each standard package size and software label preview accurately represents package size. Standard available choices for variable length packages include: 1", 1-1/8", 1-1/4", 1-3/8", 1-1/2", 1-7/8", 2 -1/4". BV-1Barcode Verifier Option - automatically scans each package as a final check after the packaging of the unit dose container is completed to assure that the barcode can be accurately scanned at the bedside. If an unreadable barcode is detected the system will pause to allow the user to retry or remove the package from production. CA-SCAN42D USB Barcode Scanner - adds the ability to scan the bulk container at packaging time to streamline and safeguard the packaging process. INSTALL On-site Installation and Training Period of delivery shall be: 6 weeks NOTICE TO OFFERORS: Instructions for submitting quotations under this request for quote must be followed. Failure to provide all information to aid in the evaluation may be considered non-responsive. Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. In addition to written price quotes, offers are instructed to provide: Submit quotations via email at or before the time specified in the solicitation. Email quotes are acceptable and can be sent to brooke.hansen@va.gov. All questions must be submitted in writing (via email) no later than 5:00pm on Friday. At a minimum the contractor shall provide the following information on their quote: a) Point of contact name, telephone and E-mail address. b) DUNS Number New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Quote MUST be good for 90 calendar days after close of solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. The full text of the referenced FAR clauses may be accessed electronically at https://www.acquisition.gov/far/. FOB Destination CONUS (Continental US). No partial shipments unless otherwise specified. New equipment ONLY. NO remanufactured products. Quotations are to be submitted as all or none. No late quotes will be accepted. Award will be made to the responsible Offeror, whose offer conforming to the solicitation, will be the most advantageous to Veterans Affairs on the basis of price, technical capability and delivery schedule.
