- This is a request for information only. The information provided in response to this notice will assist the Government in determining the extent of the small business and socio-economic market. In the event small business contractors are not available for adequate competition, the requirement will be advertised as unrestricted. The primary type of work will include medical and dental non-clinical case management and administrative assistant/care coordinator staffing in support of Individual Medical Readiness (IMR) for the New Jersey Army National Guard. Any resulting solicitation will be issued under the North American Industry Classification System (NAICS) code 621498 with a small business size standard of $20.5 million average annual revenue for the last three fiscal years. It should be noted that this requirement may be solicited by using either the Federal Business Opportunities (FBO) or by GSA E-Buy. A final determination has not been made as of this date. Award will be firm fixed price for one year with two one year option periods. The primary place of performance will be Joint Base -McGuire/Dix/Lakehurst, New Jersey and the national Guard Training Center, Sea Girt, New Jersey, however, travel to other locations within the New Jersey may be required. Responses shall not exceed two (2) typewritten pages and shall include the following information: (1) Positive statement of intent to quote as a prime contractor, (2) Identification of firm with the assigned Dun and Bradstreet (D&B) Data Universal Numbering System (DUNS) number, Commercial and Government Entity (CAGE) code and appropriate socio-economic status, (3) A listing of projects completed during the past three years, both government and private industry. This list should include the project, dollar value, contract number, location and point of contact. In addition, indicate if performance was as the prime or subcontractor for each project. (4) A brief outline of resources, subcontractors and key personnel available for use to accomplish work.
