Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation is for medical equipment that is listed below, the solicitation number is VA247-15-N-0254, and this solicitation is a Request for Quote (RFQ). The associated North American Industrial Classification System (NAICS) code for this procurement is a 532490 with a small business size standard of $32.5 million. Only qualified Offerors may submit quotes. 1)Wound VAC Therapy Bed Rental Quotes shall be evaluated on the basis of Lowest Price Technically Acceptable (LPTA) criteria. Shipping shall be by F.O.B. Destination. Vendors must honor quotes for 30 days from the closing date of this RFQ. Please complete the blanks in the Schedule/Specifications chart, and include any local part numbers. Quotes shall be emailed to Takya.smith@va.gov. Please include "VA247-15-N-0254" in the subject line of emails. Technical inquiries can be emailed to Takya.smith@va.gov. Technical inquiries will be accepted through 12 noon EST, on March 12, 2015. For technical inquiries, please ensure that the subject line of all emails include "VA247-15-N-0254." Telephone requests for information or questions will NOT be accepted. No hard copies of this RFQ will be provided. *** All Request for Quote's (RFQ's) should be submitted by Friday, March 20, 2015 at 12:00 NOON EST via email to takya.smith@va.gov. No RFQ's will be ACCEPTED after this deadline. *** *** Award shall be based on the Lowest Price Technically Acceptable basis. Award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards. ***
