Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 13.303, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number VA259-15-Q-07 is issued as a request for quotation (iii) This is Brand name specific. 10ml pre-filled Syringe 5-pack per case 8 Cases total (iv) Two cases to be delivered initially. Additionally cases to be shipped upon request from Point of Contact at Salt Lake City. FOB Dest to below within 30 day ARO VAMC Salt Lake City 500 Foothill Rd Salt Lake City UT 84148 (vi) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. (vii) The applicability of the provision at 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. Evaluations will be based on Lowest Priced Techincally Acceptable (LPTA).
