Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR Part 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offeror(s) may submit offers. The Department of Veteran Affairs, Medical Center, Los Angeles California, has an urgent requirement for an Annual Service Contract for the Maintenance for the Dako - Model Artisan-Link. The resulting Contract will be a Firm-Fixed Price Agreement. The Solicitation is issued as a Total Small Business Set-Aside. The period of performance is one year. Interested parties responding to this RFQ shall provide an e-mail a response by the due date indicated on the Solicitation. Solicitation VA262-15-Q-0560 is issued as a Request for Quote (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. This procurement will be a total small business set-aside. NAICS Code is 811219 and size standard is $19 million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. This service is required for the VA Los Angeles Medical Center, 11301 Wilshire Blvd, Warehouse Bldg. 297, Los Angeles, CA 90073 within VISN 22, in accordance with the terms, conditions, special provisions and schedules herein. Award will be made to the contractor technically acceptable at the lowest price which conforms to the requirements within this solicitation. Brand name or equal quotes will also be accepted. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document. Additionally, the Contractor shall provide a minimum of three (3) Past Performance references.
