Description
The Department of Veterans Affairs, VA Boston Healthcare System, has a requirement for Nurse Abstractor services needed for research purposes. A fixed price contract with a base of September 28, 2015 through September 27, 2016 with one (1) 1-year option period is anticipated and shall be awarded. It is the Government's belief that only the selected individual possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate only with this source. This is not a solicitation; it is notice of intent to sole source a requirement. Inquiries/information received after the established deadline shall not be considered. The submission of any data for review shall not impede award of this contract as planned. The Government does not intend to pay for information solicited. This synopsis is issued pursuant to FAR 6.302-1. STATEMENT OF WORK The Center for Healthcare Organization and Implementation Research (CHOIR), an HSR&D Center of Excellence at the VA Boston Healthcare System, requires contract services of an experienced nurse abstractor to support the VA HSR&D CDA funded study, CDA 13-270 "Development of an Adverse Event Surveillance System for Outpatient Surgery" Dr. Hillary Mull (Principal Investigator and Supervisor). The mission of the Center is to conduct health services research on timely and significant questions concerning the effects of organization and management practices on quality of health care and on implementation of organizational change in VA, for the purpose of improving the quality of care for veterans. The Nurse Abstractor will perform chart review to detect adverse events among select outpatient surgeries in VA. This task will require a careful review of all components of the medical record (e.g., discharge record, admission and physical history, patient progress notes, nursing records etc). The Nurse Abstractor will have to use some clinical judgment when examining the medical record. For example, different sections of the medical record may conflict with each other and the abstractor will be required to make judgments as to which section has the correct information. The Nurse Abstractor will also have to identify processes of care and understand the context in which care is delivered. Furthermore, the Nurse Abstractor must have direct experience reviewing medical records to identify adverse events that occur in outpatient surgery. Primary duties include, but are not limited to: 1.Contribute to the revision of a standardized chart abstraction instrument. 2.Conduct medical chart abstraction from the VA Computerized Patient Record System (CPRS) and VistA. 3.Participation in project meetings to coordinate with the PI and CDA mentor team and to update the project team on findings. Specific items to be delivered to Dr. Hillary Mull, VA, by the Nurse Abstractor include, but are not limited to: 1.Written feedback/edits on study materials as requested (e.g., chart abstraction instrument). 2.Chart abstraction notes and standardized abstraction results. 3.Brief monthly reports listing the number of hours worked, a brief description of the duties performed, and the amount to be invoiced. Breakdown of the specific tasks to be performed or components to be supplied: 1.Assist team as requested in reviewing and updating study materials (e.g., chart abstraction instrument). 2.Access sample of patients for chart review through secure VA mechanism. 3.Perform thorough medical chart abstraction using a standardized abstraction instrument to detect adverse events in outpatient surgical patients. 4.Provide expert advice for troubleshooting any chart abstraction issues that arise. 5.Attend/call into monthly team meetings as requested. Of all trained clinicians, nurses are most familiar with the meaning of the terminology used in the medical record. Further, their experience uniquely qualifies them to locate the request information from the various sections of the medical record. In assessing explicit criteria, several studies suggest that nurses do this better than physicians and other clinicians. Contractor must have the following qualifications: 1.Previous experience as a nurse researcher/nurse abstractor detecting outpatient surgical adverse events 2.Extensive experience in the review and abstraction of data from VA medical records using VA CPRS and VistA 3.Extensive experience in and knowledge of research protocols and practices 4.Extensive clinical knowledge and nursing experience The Nurse Abstractor is required to submit to Dr. Mull a monthly timesheet listing the number of hours worked, a brief description of the duties performed, and the amount to be invoiced. Dr. Mull will supervise the Contractor and will monitor the Nurse Abstractor's performance. Contractor is required to meet weekly with Dr. Mull in order to provide updates on the progress of her duties and discuss any other issues relevant to the above named study. The Nurse Abstractor is to attend weekly team meetings (in person or by phone) as needed and check-in by email and phone regularly with Dr. Mull in order to provide updates on the progress of her duties and discuss any other issues relevant to the above named study. In addition, the Nurse Abstractor will be available by telephone and/or email in order to provide updates on the progress of her duties and discuss any other issues relevant to the above named study. The Nurse Abstractor will perform her duties remotely or at the VA Boston, Center for Healthcare Organization and Implementation Research (CHOIR) Building 9 on the Jamaica Plain campus. When the Nurse Abstractor works remotely, she will provide her own computer and will be responsible for securing One VA-VPN/Remote Access (Citrix). At times, the Nurse Abstractor will perform her duties at the VA Boston Healthcare System, CHOIR, Building #9, Jamaica Plain Campus where she will be provided with a work space to conduct the medical chart abstractions. The Nurse Abstractor will be provided a secure location to store any sensitive VA data/information allowing her to engage in safe data management practices. The Nurse Abstractor will need access to the VA CPRS, VistA, and One VA-VPN/Remote Access (Citrix). The Nurse Abstractor is required to complete all mandatory research trainings, including but not limited to, human subjects, data security, and privacy trainings, and ensure that these trainings are current in accordance with VA policy. Some specific guidelines that the Nurse Abstractor must have knowledge of when performing work are: VHA Handbook 1200.05, VHA Intellectual Property Handbook 1200.18, and VA Cyber Security and Privacy Policies (VHA Handbook 1605.1, VA Directive and Handbook 6102, VA IT Directive 06-2, VHA Directive 6500, VA Directive 6502, VA Handbook 6502.1 and 6502.2, and VA Directive 6504). The Nurse Abstractor will perform the work according to a schedule as permitted by Dr. Mull, Principal Investigator and will respond to Dr. Mull's inquiries regarding the work to be performed and/or other study issues within a reasonable time frame. The frequency of how many times a certain task must be performed and the schedule as to which the tasks are to be performed will vary.
