- SF-1449 Statement of Work Hazard Tree Removal Project Black Hawk Park 1. SCOPE: The work will consist of cutting, grinding and removal off site* of twenty-seven (27) trees, as well as grinding an additional thirteen (13) stumps. All work will be completed with the park boundaries of Blackhawk Park. *Ash wood debris must meet WI DATCP and USDA requirements. Contractors are responsible to make sure they meet all requirements for handling and hauling ash wood debris. Note: Vernon County is a quarantined county. It is best to contact the USDA and/or WI DATCP. 2. PROJECT LOCATION: The project site is located between Highway 35 and the Mississippi River in Vernon County at: U.S. Army Corps of Engineers, Blackhawk Park, E590 County Road BI, DeSoto, Wisconsin, 54624. 3. WORK SCHEDULE: Contractor shall harvest and remove all trees and debris before 01 April 2015. All stump grinding and back filling shall be completed by 15 May 2015. 4. SUBMITTALS The Contractor shall submit the following as required. Receipt of submittals by the Government will not relieve the Contractor of responsibility to furnish materials or perform the work as required. A. Certificate of Insurance: The Contractor shall submit proof of insurance for Workman Compensation and Liability within 10 days after award and before any work can begin. All insurance shall be adequate to indemnify the Government from liability for injury to the Contractor's employees and damage to Government property. B. Safety Plan: A written statement of intended work methods, procedures, equipment, and materials to be used, indicating how the Contractor will comply with all safety requirement, as listed below in Section 7. 5. SAFETY: The Contractor is to comply with all safety and health requirements as outlined in the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, dated November 2014, to insure safe working conditions for contractor employees and government visitors to the site. CONTRACTOR AND GOVERNMENT APROVIDED MATERIALS: The Contractor will provide Government approved topsoil, and grass seed. Topsoil will be reasonably free from clay, roots, stones, brush, and other materials. The Government will inspect and approve all materials prior to installation. 7. IMPLEMENTATION: A. Notification of Government Point of Contact (POC): Contractor shall provide the POC a work schedule at least 5 days before work begins. B. Harvested Trees: An on-site meeting prior to harvesting will be held to discuss operational concerns about harvesting. Roads and ground used must be repaired to pre-work conditions. Any neighboring tree damaged while harvesting will be repaired to a safe condition. The Contractor is responsible for removing all harvested trees and tree waste off site. The twenty-seven (27) trees to be harvested will be marked with an X- on the tree. Below are the number of trees to be harvested listed by the circumference of the tree at chest height. Note: The location of some trees are adjacent to water or possibly overhanging water. Circumference Number of Trees 15 inches - 29 inches 6 30 inches - 60 inches 14 70 inches - 135 inches 7 C. Stump Grinding The Contractor is responsible for grinding all stumps from trees harvested listed above, as well as an additional thirteen (13) stumps. Of these additional thirteen, eight are less than 50 inches in circumference, and five are between 50 and 85 inches in circumference. This will total 40 stumps to grind. Grinding must be done to a depth of 12 quote mark below ground, and must be backfilled with topsoil flush with ground level and seeded with grass seed. 8. PRE-WORK MEETING A pre-work meeting will be held shortly after award. Quality control and inspection procedures will be reviewed at that time. The Contractor will be notified by the Corps of Engineers of the meeting location and time. 9. GOVERNMENT LIABILITY: The Government assumes no liability from the contractor's performance of the contract work. The contractor shall hold and save the Government harmless from any and all liability to others arising from the performance of the contract. 10. RESPONSIBILITY FOR DAMAGE: The contractor is responsible for all damages to persons and property resulting from performance of the contract work. Excess and waste material shall be removed and properly disposed of off-site.
