- The Northern Region Contract Office, Womack Army Medical Center (WAMC) Fort Bragg, NC 28310 has the requirement for Bereavement Training Course (or equal training) as outlined in the Performance Work Statement (PWS) below. If your company can provide Bereavement Training Course (or equal training) meeting all the requirements of the PWS please respond to this Sources Sought. Responses are to be emailed to Mary Mitchell Martin (mary.a.mitchellmartin.civ@mail.mil) no later than 23 March, 2015, 8:00 AM EST. Telephone calls will not be accepted nor responded to. PERFORMANCE WORK STATEMENT To provide one session of the two-day Training Course in Perinatal Death and one-day Coordinator Training, and two-day Bereavement training course in Pediatric and Adult Death at Fort Bragg, NC on May 4-8, 2014 1. GENERAL: This is a non-personnel services contract to provide bereavement training. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. 1.1. BACKGROUND: 1.2. SCOPE OF WORK: The Contractor shall provide qualified personnel, services, and materials necessary to perform bereavement training. 1.2.1. The Contractor shall comply with all applicable laws, rules and regulations, including but not limited to those included in Section 6 of the PWS. 1.3. SAFETY REQUIREMENTS: The Contractor shall maintain safety and health standards compliant with requirements of the Occupational Safety and Health Administration (OSHA). 1.4. SECURITY REQUIREMENTS: The Contractor shall be responsible for the security of all sensitive information. 1.5. PERFORMANCE QUALITY: The Contractor shall establish and maintain a complete Quality Assurance Surveillance Plan that shall ensure the requirements of the contract are provided as specified in Section 5 of the PWS. Within thirty calendar days after contract award, three copies of a comprehensive written QASP shall be submitted to the COR and within 5 working days when changes are made thereafter. 2. DEFINITIONS AND ACRONYMS: 2.1. DEFINITIONS: 2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract. 2.1.5. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards. 2.1.7. SUBCONTRACTOR. One that enters into a subcontract and assumes some of the obligations of the primary Contractor. 2.2. GOVERNMENT FURNISHED ITEMS AND SERVICES: 3.1. MATERIALS. 3.1.1. The Government will provide: a)Windows XP or later operating system b)Microsoft PowerPoint 2010 c)500 MHz processor or higher d)512 MB of RAM e)Display capable of 1024x576 f)5 microphones, two stools, flip board and markers g)Audio speakers connected to the PA system h)Projector and screen 3.2. FACILITIES. 3.2.1. Provide one room for Conference meeting the following specifications: a)Capacity for up to 45 participants for Bereavement Training in Perinatal Death (days one and two), and up to 20 participants for Coordinator Training (day three), to 45 participants for Bereavement Training Pediatric and Adult Death (day four and five) b)Classroom-style setting with tables and chairs c)Private (i.e., no walk through or common area that would cause disruptions). 3.3. GOVERNMENT SERVICES. 3.3.1. Arrange for person(s) to set up audio visual equipment prior to 0700 on the first day of Conference, and be available to trouble shoot audio visual problems during Conference. 3.3.2. Arrange for person(s) to set up a conference room prior to Conference start date, including: a)Table and chair set up b)Placement of manuals on tables 3.3.3. Arrange for person(s) to assist with registration and sign-in each day of the Conference 3.3.4. Ship all conference materials previously shipped to the Government's designated location back to the Contractor upon completion of Conference 4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under section 3 of this statement. 4.1. PERSONNEL QUALIFICATIONS. An outline of qualifications for the Contractor staff is given below: 4.1.1. Qualified and knowledgeable speaker(s) to present the Resolve Through Sharing Training. 4.2. MATERIALS. The Contractor shall provide: a)Access to three online modules: Caring for the Family Experiencing Perinatal Death, Communicating with the Bereaved, and Theoretical Framework of Grief (or equal modules). b)Latest edition of the Bereavement Manual, lapel pin, contact hour certificate, and color certificate for all participants completing Bereavement Training in Perinatal death (or equal modules). c)Latest edition of the Coordinator Manual, contact hour certificate, and certificate of completion for all participants completing Coordinator Training (or equal modules). 4.2.1. Furnish materials, supplies, and equipment necessary to meet the requirements for the provision of services established under the terms of this contract. 5. DESCRIPTION OF WORK: 5.1. Contractor shall provide the following for the conference: a)Provide speaker(s) for the Conference. b)Offer contact hours for nursing, social work, and chaplaincy/clergy. c)Provide access to three online modules (or equal modules), quote mark Caring for the Family Experiencing Perinatal Death quote mark, quote mark Communicating with the Bereaved quote mark, and quote mark Theoretical Framework of Grief. quote mark Completion of these modules is a prerequisite to attending the classroom portion of Conference. d)Provide the latest edition of the Bereavement Manual, lapel pin, contact hour certificate, and color certificate of completion for each participant completing Bereavement Training in Perinatal Death. e)Provide the latest edition of the Coordinator Manual, contact hour certificate, and certificate of completion for each participant completing- Coordinator Training. f)Ship all conference materials to the address provided by the Government. 6. APPLICABLE LAWS, REGULATIONS, AND MANUALS: (Current Editions) 6.1. If Section 952 of the Omnibus Budget Reconciliation Act of 1980 and the regulations promulgated thereunder are applicable to this Agreement, Bereavement Services shall, until four (4) years after the expiration of this Agreement, comply with all requests by the Comptroller General of the United States, the Secretary of the Department of Health and Human Services, and their duly authorized representatives for access to this Agreement and to the parties' respective books, documents and records necessary to verify the nature and extent of the costs of materials and services provided. Such access shall be requested in accordance with section 952. A provision similar to this paragraph shall be included in any contracts between a party and its subcontractors, if any, for services provided hereunder. 6.2. To the extent applicable to this Agreement, Bereavement Services agrees to comply with the Health Insurance Portability and Accountability Act of 1996, as codified at 42 U.S.C. Section 1320d ( quote mark HIPAA quote mark ) and any current and future regulations promulgated hereunder, including, without limitation, the federal privacy regulations contained in 45 C.F.R. Parts 160 and 164 ( quote mark Federal Privacy Regulation quote mark ), the federal security standards contained in 45 C.F.R. Part 160, 162, and 164 ( quote mark Federal Security Regulation quote mark ), and the federal standards for electronic transactions contained in 45 C.F.R. Parts 142, 160, 162 (the quote mark Federal Electronic Transactions Regulations quote mark ), and the Health Information Technology for Economic and Clinical Health Act ('HITECH') contained in Section 13402 of Title XIII of the American Recovery and Reinvestment Act of 2009 ( quote mark Stimulus Bill quote mark ), as applicable, all as may be amended from time to time. 6.3. Comply with all applicable federal, state and local statutes, rules and regulations, licenses and authorizations of any governmental or public body or authority in connection with the performance or carrying out of their respective obligations hereunder 6.4. Apply for, obtain and maintain any and all licenses, certifications, authorizations and permits necessary to carry out the provisions of this Agreement
