Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. The solicitation number is VA249-14-R-0957 and is issued as a request for proposal (RFP). This requirement is a Full and Open Unrestricted Competition. The associated North American Industrial Classification System (NAICS) code for this procurement is 621399 with a small business size standard of $7.5 million. To be considered for award of a Government contract, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. The Department of Veterans Affairs, Network Contracting Office 9 is soliciting proposals for Perfusionist Services at the Tennessee Valley Healthcare System, Nashville Campus. This RFP requires the provision of perfusionist services as-needed, per case. The anticipated period of performance will be from the date of award through a one year base period, with four, one (1) year option renewal periods. Option periods are exercised at the sole discretion of the Government. The government anticipates awarding a Firm-Fixed Price/ Indefinite Delivery, Indefinite Quantity (IDIQ) contract. The date for receipt of offers is April 7, 2015, 2:00 p.m. Central Standard Time. Prospective offerors should not expect notification of issuance of amendments other than via the Federal Business Opportunities website and are responsible for obtaining all amendments as posted. Questions from potential offerors are encouraged to resolve any possible ambiguities; inquiries shall be in writing only and addressed to Angela Bailey at angela.bailey5@va.gov. by March 24, 2015, 12:00pm. Phone calls will not be accepted.
